Assistant Team Lead RN

Position Summary

Serenity Hospice Care is seeking a compassionate, experienced, and organized Assistant Team Lead RN to support the clinical leadership team and provide guidance to our hospice nursing staff. The Assistant Team Lead RN will assist with the day-to-day coordination of clinical services, support RN Case Managers, help ensure quality patient care, and promote compliance with hospice regulations, company policies, and clinical standards.

This position combines direct patient care, clinical support, staff mentoring, and leadership responsibilities and is ideal for an experienced hospice RN who is ready to take the next step into a leadership role. This position will oversee Bergen/Morris County.

Responsibilities

• Assist the Team Lead with daily clinical operations and coordination of the hospice nursing team.

• Provide guidance, support, and mentorship to RN Case Managers and other clinical staff.

• Assist with assigning and coordinating patient visits based on patient needs, staffing, and geographic coverage.

• Monitor patient care to promote continuity, quality, and compliance with the hospice plan of care.

• Assist nurses with clinical questions, challenging patient situations, symptom management, and changes in patient condition.

• Participate in patient care and make nursing visits as needed to support the clinical team.

• Assist with reviewing clinical documentation for completeness, accuracy, and compliance.

• Help monitor timely completion of assessments, recertifications, care plans, orders, and other required clinical documentation.

• Assist with identifying changes in patient condition and ensuring appropriate communication with physicians, hospice providers, patients, and families.

• Support coordination between nursing, aides, social work, chaplain, volunteers, providers, and other members of the interdisciplinary team.

• Serve as a clinical resource and role model for nursing staff.

• Participate in interdisciplinary group meetings and other clinical meetings as assigned.

• Respond appropriately to urgent clinical situations and assist with escalation of patient-care concerns.

  Qualifications

  • Current, unrestricted Registered Nurse (RN) license in the State of New Jersey.

  • Associate Degree in Nursing required; BSN preferred.

  • Minimum of 2–3 years of professional nursing experience.

  • Previous hospice, home health, palliative care, or end-of-life care experience strongly preferred.

  • Previous leadership, charge nurse, preceptor, or supervisory experience preferred.

  • Current CPR/BLS certification.

  • Valid driver's license, reliable transportation, and appropriate automobile insurance.

  • Strong clinical assessment and critical-thinking skills.

  • Excellent written and verbal communication skills.

  • Strong organizational and time-management abilities.

  • Ability to work independently while functioning effectively as part of an interdisciplinary team.

  • Proficiency with electronic medical records and standard computer applications.

    Full Time 8:30am to 4:30pm Monday through Friday
