Your Role & Responsibilities

  • Provide comprehensive care to seniors in accordance with clinical standards.

  • Participate in daily huddles. CAs tend to lead the huddles
  • Document care accurately and efficiently with support from quality-based coders.

  • Ensure compliance with all licensing and accreditation requirements.

What We’re Looking For

Required:

  • Master’s Degree in Nursing or completion of a PA program with board certification.
  • Active, unrestricted NP or PA license in the state of practice.
  • Strong communication and interpersonal skills.
  • Passion for value-based care and integrated team-based models.
  • Basic computer proficiency (EMR, email, etc.).

Preferred:

  • Experience in value-based care, primary care or Medicare Advantage settings.
  • Active DEA license and Medicare/Medicaid provider numbers or ability to obtain DEA license and/or Medicaid and Medicare numbers
  • Familiarity with HEDIS quality indicators and Medicare guidelines.

Overview: JOB PURPOSE: The APP Fellow will work within a Critical Care team under the direction of an APP and a Physician to provide comprehensive and consistent care for critically ill patients in a hospital setting. The Fellow will gain experience in assessing, diagnosing, and managing care, selecting interventions, performing required procedures, and consistently evaluating patients' progress toward medical goals. Responsibilities: KEY RESPONSIBILITIES: 1. Provides direct clinical services for critically ill adult patients with cardiac or multi-system failure, within institutional guidelines and utilizing evidence-based clinical practice standards under the supervision of the patient's physician. 2. Obtains patient health histories and completes physical examinations and documents findings in accordance with hospital requirements. 3. Orders and interprets appropriate diagnostic and laboratory tests, explaining the necessity, preparation, nature, and anticipated effects of procedure(s) to patients, patients' family, staff, and healthcare learners. 4. Performs approved therapeutic or diagnostic procedures based upon the patient's clinical status and documents procedures in accordance with hospital requirements; assists the physician with diagnostic and therapeutic procedures, etc. 5. Performs cardiopulmonary resuscitation and external cardiac massage; airway management including bag and mask ventilation, endotracheal intubation and extubation, ordering of life supporting drugs according to established protocols, and defibrillation according to Advanced Cardiac Life Support standards. 6. May perform thoracentesis, thoracostomy tube, and bronchoscopy, as well as other indicated procedures while under the direct supervision of an intensivist until independent competency is confirmed. 7. Under physician direction and within the credentialing standards of the Hospital, inserts and manages arterial catheters, central venous catheters, pulmonary artery catheters, Chest tubes, thoracenteses, endotracheal tubes, and extra glottic devices. Interfaces with patients using monitoring and instrumentation equipment, and directs and educates others in performing these functions. 8. Formulates and implements a treatment plan for acute illness management in collaboration with the attending physician(s) and documents in medical records and appropriate narrative summaries. 9. Assesses patients for changes in status and institutes appropriate interventions, including initiation of necessary emergency interventions to stabilize the patient. 10. Incorporates clinical research findings in the development and implementation of standards of care. 11. Supports IRB-approved clinical research/trials through a variety of activities, including but not limited to: obtaining consent; ordering diagnostic tests; recording laboratory and diagnostic results; administering therapeutic interventions; and documenting and reporting patient outcomes, including toxicities or adverse events. 12. Performs and participates in quality/performance improvement activities and clinical research. 13. Serves as a clinical expert and education consultant to nursing, members of the healthcare team, and the larger community. 14. D-SNP Nurse Practitioners will visit members in their home as well as conduct telehealth visits for health risk assessments for new and post care transition members. Nurse Practitioners have an advanced nursing practice that includes independent nursing functions based on nursing standards of care and a role in medical management working within a collaborative agreement with a physician(s). KEY RESPONSIBILITIES: 1. Proven ability to perform comprehensive, adult/geriatric admission history and physical exam 2. Demonstrates ability to identify, diagnose and document episodic, acute and chronic disease in adults 3. Demonstrates ability to recognize conditions, characteristics, and behaviors that place seniors at risk 4. Proven ability to pursue feedback, continuing education and professional development 5.
