As a Discharge Care Manager LPN on the IHCI Team, you will have the opportunity to have a profound impact on the lives of people living with complex and/ or chronic conditions. The Discharge Care Manager LPN connects with the patients on the phone, or in person, in the physician offices, or in the hospital setting. The IHCI LPN Post-Discharge Care Manager will be providing telephonic with some face-to-face interaction with patients and their care team as appropriate to improve patient care. Along with other members of the Population Health team, conduct patient surveys that include the medical, behavioral, pharmaceutical and social needs of the patient, identify gaps in care and barriers to attaining improved health. Based on this patient survey, and in conjunction with the patient, the patient’s physician and other members of the population health team manage a care plan that will address the identified needs, remove the barriers and improve the health of the patient. Coordinate care by serving as the contact point, advocate and resource for the patient, their family and their physician, building effective relationships through trust, respect and communication. In close collaboration with the patient, primary care provider and care team you will continually monitor the patient’s knowledge of their clinical condition(s) and provide education and self-management support based on the patient’s unique learning style. The Discharge Care Manager LPN will measure, improve and maintain quality outcomes (clinical, financial, and functional) for individual patients and the population served.

Exceptional Skills And Qualifications

  • 2 year / associate degree in practical nursing. (required)
  • Licensed as a Licensed Practical Nurse (LPN) with a valid license to practice in the state of Indiana as listed in the Nurse Licensure Compact (NLC). May require Illinois and/or Kentucky licensure in the future. (required)
  • 3+ years: Experience in home health, ambulatory care, community public health, case management, or coordinating care across multiple settings and providers. (required)
  • Population Health management experience. (preferred)

Make a Difference Responsibilities As a Remote Patient Monitoring LPN ( Post-Acute Care Coordinator LPN), you will support patients enrolled in a remote monitoring program by reviewing transmitted health data, providing patient outreach, and escalating concerns according to established clinical protocols. This role works closely with nursing leadership and care teams to promote early identification of health changes, improve chronic disease management, and enhance patient engagement—all in a remote, technology‑enabled care model. Review and respond to care path focused questionnaires submitted by patients. Conduct outbound phone calls or secure messages to patients to assess symptoms, reinforce education, and support adherence to care plans Document patient interactions, clinical assessments, and interventions accurately in the electronic health record Communicate effectively with providers and their triage team, pharmacy, and other care team members to coordinate follow‑up care as needed Educating patients on proper device usage, data transmission, and self‑management strategies Reinforcement of care path zones based on vital sign trends Support quality, compliance, and safety initiatives related to remote patient monitoring services Maintain patient confidentiality and comply with HIPAA and organizational policies Exceptional Skills And Qualifications Graduate of a recognized accredited school of practical nursing, or Graduate from a non-accredited school of practical nursing with three years related professional nursing experience required.
