Responsibilities:

Provide direct patient care, including administering medications, wound care, and vital sign monitoring
Collaborate with other healthcare professionals to develop and implement individualized care plans
Maintain accurate and up-to-date records of patient care and treatment
Participate in care conferences and multidisciplinary team meetings to ensure comprehensive care
Assist with daily living activities, such as grooming, bathing, and transferring patients

Requirements:

Current LPN license in the state of Massachusetts
Ability to work independently and as part of a team
