Job Summary:

We are seeking a dedicated and experienced LPN/RN to work in a fast-paced and dynamic long-term care environment. As an LPN/RN at The Center at Blue Hills, you will be responsible for providing high-quality nursing care to our residents, managing medications, observing and reporting changes in resident condition, and collaborating with other healthcare professionals to ensure comprehensive care.

Responsibilities:

Provide direct patient care to residents, including assessments, medications, and treatments
Develop and implement individualized care plans in collaboration with the interdisciplinary team
Monitor and report changes in resident condition to the physician and other healthcare professionals
Maintain accurate and up-to-date records of resident care and treatment
Participate in quality improvement initiatives and maintain ongoing education and training
