Job Summary:

We are seeking a compassionate and dedicated Full-Time Caregiver to join our team at SYNERGY HomeCare in Bedford, NH. As a Full-Time Caregiver, you will play a vital role in providing high-quality care and support to our clients, enhancing their quality of life, and helping them feel safe and comfortable in their own homes. Responsibilities:

Provide personalized care and support to clients, including assistance with daily living activities, medication management, and mobility and transfer assistance
Establish and maintain positive relationships with clients, families, and healthcare professionals
Conduct initial and ongoing assessments of clients' needs and preferences to develop individualized care plans
Administer medications, treatments, and other therapies as instructed
Maintain accurate and detailed records of client care and services
Communicate effectively with clients, families, and healthcare professionals to ensure seamless care coordination
Participate in ongoing training and education to stay current with industry best practices and industry regulations
