Licensed Practical Nurse (LPN) - Full-Time

Responsibilities:

  • Provide direct patient care, including vital signs, wound care, and medication administration

  • Conduct initial and ongoing assessments of patients' physical and psychological needs

  • Develop and implement individualized care plans in collaboration with the interdisciplinary team

  • Administer medications and treatments as prescribed by physicians

  • Document patient information accurately and thoroughly in accordance with facility policies

  • Collaborate with other healthcare professionals to ensure seamless and efficient care delivery

  • Participate in quality improvement initiatives to enhance patient outcomes and satisfaction

Requirements:

  • Current LPN license in the state of Pennsylvania

  • Basic Life Support (BLS) certification

  • Ability to work in a fast-paced environment with flexibility and adaptability

  • Strong communication and interpersonal skills

  • Ability to lift, push, and pull patients as needed

Licensed Practical Nurse - Acute Rehab - Per Diem

Position Summary: Provides high quality nursing care for assigned patients in accordance with nursing standards of care and practice under the supervision of a Registered Nurse. Maintains collaborative relationships with other nursing staff members, physicians and ancillary staff to provide this care. Maintains an appropriate level of knowledge and clinical skill through continuing education/in-services and self‐study activities. Education, License & Cert: LPN - Must be licensed in the state of Practice Experience: For those that will be working in PA, the individual must have completed the IV therapy curriculum requirements as noted in the Pennsylvania Code, Section 21.203. If not completed at the time of hire, the individual must complete a Board‐ approved, LPN IV Therapy Education Program prior to independently practicing IV therapy. In addition, the staff LPN must meet individual nursing unit specifications regarding educational requirements during employment. Must maintain CPR certification. Essential Functions: 1. Works collaboratively with and under the direction of a Registered Nurse. A. Demonstrates the ability to communicate observations of patient to members of the healthcare team. B. Supports the philosophy and objectives of the nursing department. 2. Assists the Registered Nurse with nursing process. A. Participates in patient data collection, including the admission process. B.
