Job Overview

The Clinical Nurse Supervisor, AKA Associate Clinical Manager (ACM), provides frontline clinical and operational leadership to ensure the delivery of safe, high-quality, and efficient patient care. Through direct clinical care and support, visible leadership presence, and active supervision of team members, the ACM drives excellence in care delivery, workforce performance, and patient/family experience while upholding regulatory and organizational standards.

This position provides leadership and support to Case Managers and Discharge Planners, partners closely with providers and interdisciplinary teams, and helps advance patient progression and discharge planning initiatives across the organization. The ACM plays a key role in supporting staff development, resolving complex patient and operational escalations, and fostering a culture of accountability, collaboration, and continuous improvement.

The Case Management ACM supports daily operations, staffing effectiveness, and strategic initiatives while helping ensure high-quality care coordination and efficient patient transitions. This role offers an opportunity to influence department operations, mentor team members, and contribute to the ongoing growth and evolution of Case Management services.

Duties & Responsibilities

  • Provides clinical leadership and management, oversees operations, and directs staffing support to ensure high-quality patient care, including serving in charge or other clinical roles, demonstrating clinical expertise, and reinforcing professional standards and competencies.
  • Maintains leader visibility through rounding with patients, families, and team members to assess care delivery, ensure timely resolution of identified issues, promote positive experiences, and recognize best practices.
  • Ensures compliance with policies, procedures, and guidelines, in alignment with evidence-based practice, regulatory requirements, and organizational standards.
  • Ensures safe, efficient patient flow and resource utilization.
  • Creates and manages staffing and scheduling resources to meet patient, operational, and departmental needs, adjusting resources as conditions change.
  • Supports interviewing and hiring decisions, and oversees onboarding and training activities, providing input and recommendations in collaboration with departmental leadership.
  • Develops, coaches, mentors, and directly supervises team members, including directing their work, supporting performance expectations and professional growth, handling employee complaints and grievances, conducting performance reviews, corrective action and discipline, and recommending termination decisions, where needed.
  • Supports continuous improvement initiatives that enhance clinical practice, operations, and patient outcomes.
  • Monitors departmental resources and expenditures and assists with meeting targets for productivity, efficiency, and waste reduction.
  • Provides direct clinical care and support, visible leadership presence, and active supervision of team members.

Minimum Qualifications

  • EDUCATION – Bachelor’s degree in Nursing. Master’s degree in Nursing or a directly related health care field is preferred.
  • EXPERIENCE – Three years of clinical experience. Case Management/Care Coordination experience preferred.
  • LICENSURE – Registered Nurse (RN)
  • CERTIFICATION(S) – BLS/CPR from the American Heart Association with at least 6 months left before expiration is required upon hire. ANCC recognized certification in clinical specialty. Depending on the unit/department, employees may also be required to have a Pediatric Advanced Life Support (PALS), Adult Advanced Life Support (ACLS), and/or Emergency Nursing Pediatric Course (ENPC).

Salary Information

Pay is dependent on applicant's relevant experience.

Annual Salary Range (Based on 40 hours worked per week): $108,198.40 to $162,297.60

Support the continuity of care by recognizing and supporting home health, palliative and hospice referrals as appropriate and recognizing and supporting transitions in levels of care. Document timely clinical records on all patients including screening/assessment findings, physician orders, progress notes, and care plans. Maintain effective working relationships with IDG, and involve team when patient's care plan warrants team participation to achieve desired outcome. Participate in on-call and patient support activities as applicable, conducting on-call services in a clinically competent and responsive manner. The requirements listed below are representative of the knowledge, skill, and/or ability required. Education: Registered Nurse Experience: A minimum of one (1) year experience as an RN, either in acute care hospital, oncology, long-term care, hospice or home health. School internships and clinical hours may be considered as experience in individual cases. Hospice or palliative care experience preferred Licenses, Certifications and/or Registration: Current license to practice nursing in the state of requested employment CPR Certification Current automobile insurance and valid driver’s license Specialized Knowledge and Skills: Knowledge of terminally ill patients and their families along with understanding of hospice; ability to apply knowledge of the special needs of hospice patient and families; knowledge of roles of all disciplines providing hospice services; excellent patient assessment skills; knowledge of community resources; good oral and written communication skills; documentation and program management; knowledge of general nursing practice; thorough knowledge of managed care principles, regulatory guidelines (i.e., Medicare, Medicaid, ACHC, and human resource management principles). Working Conditions: Patient’s homes and/or institutional setting and automobile. Potential exposure to infections, communicable diseases, odors, blood, excreta and hazardous materials.
