Hospice Patient Care RN Manager

We are seeking a dedicated Patient Care Manager to join our hospice team. Reporting directly to the Executive Director or Administrator, you will oversee patient care activities to ensure the delivery of high-quality, compliant hospice services. This leadership role involves managing clinical operations, coordinating care, and developing staff to uphold the highest standards of patient care and regulatory compliance.

As a Hospice Patient Care Manager, You Will:

  • Oversee and direct patient care services to ensure the delivery of high-quality hospice care

  • Manage visit scheduling, including reassigned, missed, declined, or rescheduled visits—making decisions based on clinical judgment and priority

  • Triage incoming phone referrals and medical concerns in a timely, compassionate, and professional manner

  • Lead or facilitate Interdisciplinary Group (IDG) meetings, including presenting patient updates, reviewing charts, coordinating care plans, and documenting minutes

  • Serve as a mentor, trainer, and clinical support to associates, supporting development, performance, and compliance

  • Ensure all care delivery meets federal, state, and local regulatory standards, as well as company policies and procedures

  • Support performance improvement initiatives, quality assessments, and operational efficiencies within your location

About You

Qualifications – What You’ll Bring:

  • Current RN license in the state of employment

  • Graduate of an accredited nursing program

  • Minimum of 3 years of direct patient care experience, including 2 years in a hospice or home-based care setting

  • Demonstrated ability to apply hospice principles, industry standards, and regulatory compliance (Medicare, Medicaid, JCAHO, ACHC)

  • Knowledge and compassion for terminally ill patients and their families

  • Ability to lead a diverse care team while adapting to the emotional and clinical needs of patients from various backgrounds

  • Strong interpersonal, organizational, and leadership skills

  • Competency in clinical documentation and technology systems

Preferred Qualifications (Not Required):

  • Bachelor of Science in Nursing (BSN)

  • Previous leadership, team management, or supervisory experience in hospice or home health

  • Familiarity with EMR systems and telehealth platforms

  • Certification in Hospice and Palliative Nursing (CHPN)

Compensation may vary within the salary range provided based on several factors, including but not limited to a candidate's geographic location, experience, education, skills, licensure, certifications, internal equity, and department equity. Gentiva provides associates with a comprehensive Total Rewards package, of which base pay is just one component.

Salary Range: $99,000.00 - $110,000.00

The Neuroscience Institute’s primary neurology ambulatory clinic is located on the Anschutz Campus, with additional neurology services supported across several Network of Care sites in the Denver Metro region. Our team provides comprehensive neurologic care to pediatric patients with a wide range of acute, episodic, and chronic neurological and neurodevelopmental conditions. Neurology ambulatory nurses collaborate closely with an interdisciplinary healthcare team that includes advanced practice providers, registered nurses, medical assistants, EEG/EMG technologists, social workers, and pediatric neurologists, including epileptologists, neuroimmunology specialists, neuromuscular specialists. Together, we coordinate care for patients across the continuum while supporting both diagnostic evaluations and ongoing disease management. Our clinic cares for children with conditions including, but not limited to: Chronic neurologic conditions such as epilepsy, neuromuscular disorders, cerebral palsy, neurogenetic syndromes, movement disorders, and neuroimmunologic diseases Neurodevelopmental and congenital disorders including developmental delay, autism spectrum disorder with neurological comorbidities, neurometabolic and neurogenetic disorders Acute and episodic concerns such as headaches and migraines, seizures, tics, and other new-onset neurological symptoms Duties & Responsibilities Develops and communicates the department’s strategic plan and annual goals, aligning team priorities with organizational objectives to enhance clinical practice, quality of care, and professional growth. Selects, trains, and evaluates department staff, creating a supportive environment that promotes development, engagement, and high performance. Makes decisions on hiring, promotions, disciplinary actions, and terminations in coordination with Associate Clinical Managers. Develops departmental budget recommendations and monitors expenditures to support cost-effective operations. Directs and manages daily departmental operations, ensuring adherence to policies, procedures, and regulatory standards. Uses evidence-based practice to develop and maintain guidelines for safe, effective care. Provides clinical expertise, guiding team members in delivering developmentally appropriate care and evaluating outcomes for infants and children with complex health needs. Oversees staff and financial resource allocation, adjusting as needed to meet patient care demands while maintaining budget compliance. Establishes department performance metrics and collaborates with Associate Clinical Managers to achieve clinical, operational, and developmental goals. Identifies and acts on opportunities for continuous improvement. Resolves escalated, complex, or sensitive patient/family issues and operational concerns, making inclusive, data-informed decisions that address root causes. Interprets and enforces quality and safety standards, hospital policies, and regulatory requirements to ensure compliance and promote best practices. Leads and supports a culture of clinical inquiry by mentoring team members on practice-based research and quality improvement projects. Represents the department in senior leadership and interdepartmental meetings, fostering collaboration across teams and participating in organizational committees. Minimum Qualifications Degrees Required Bachelor of Science in Nursing Master of Science in Nursing Experience Minimum Experience Required:Three (3) years of clinical experience and two years of nursing leadership experience in supervision, leading clinical practice projects, serving as a charge nurse or house supervisor, and/or precepting.
