Compensation may vary within the salary range provided based on several factors including but not limited to a candidate’s location, experience, education, skills, licensure, certifications and department equity. Gentiva provides associates with a comprehensive benefits and total rewards package, of which base pay is just one piece.

Salary Range – $18.35 - 21.60

Essential Functions

  • Provide direct personal care to terminally ill patients

  • Assist with personal hygiene, grooming, and toileting

  • Support safe mobility, including use of walkers and wheelchairs

  • Help with meal preparation, feeding, and light housekeeping

  • Observe and report changes in the patient’s physical or mental condition

  • Offer emotional and psychological support to patients and families

  • Maintain accurate and timely documentation of visits

  • Attend team meetings and participate in Quality Improvement (QI) activities

  • Practiceinfection control and follow safety protocols at all times

About You

Qualifications – What You’ll Bring:

  • Active CNA license in the state of employment (Required)

  • Current CPR certification (Required)

  • Driver’s license and reliable vehicle with insurance (Required)

  • Six (6) months of direct patient care experience (Required)

  • Comfortable working in home settings, hospice facilities, or long-term care environments

  • Demonstrated compassion, patience, and ability to handle emotional situations

  • Willingness to work independently and adapt to changing patient needs

Preferred Background (Not Required):

  • Experience in home health, hospice, palliative care, or oncology

  • Prior roles in hospital nursing, ICU, geriatrics, med-surg, ER, telemetry, or nursing homes

  • Exposure to end-of-life care, wound care, or admissions support

Assists customers with packing personal possessions when they are transferred to a new room or when being discharged. Participates in and receives the nursing report upon reporting for duty. Makes occupied and unoccupied beds, changes bed linens, and straightens rooms, closet, and drawers. Notifies housekeeping if mattress in need of cleaning or replacement. Does complete check of skin condition when dressing, undressing, turning/repositioning, etc. Reports any skin abnormalities to the Staff Nurse. Observes customer’s physical condition, attitude, reactions, appetite, etc., and reports any changes and/or unusual findings to the Nurse/RCC so care plan can be updated. Performs personal services for customer’s (i.e., reading, writing letters, etc.) as time allows. Encourages customers to help themselves and promotes self-care in aiding rehabilitation. Assists with care of dying and expired customers. Records resident food/fluid intake. Completes meal monitor form as indicated. Reports changes in resident’s eating habits to the Staff Nurse. Keeps customers water pitchers clean and filled with fresh water on each shift unless contraindicated. Serves between meals and bedtime snacks. Wears transfer belt at all times and utilizes as needed. Keeps the nurses’ call system within easy reach of the resident. Reports all hazardous equipment and conditions to the Staff Nurse immediately. Participates in the development, maintenance, and implementation of the facility’s Quality Assurance program for the Nursing Department. Performs other related duties as assigned.
