Job Summary

Coordinates a interdisciplinary plan of care or program to meet the needs of a specific patient population. Functions as a clinical, patient, and community resource, educator, and consultant.

Essential Functions

  • Participates in patient care activities for the patient population(s) served including direct patient care, indirect patient care, patient rounds, consultation, multidisciplinary care planning, and evaluation.

  • Collaborates with other health team members to develop, implement, and evaluate quality programs to meet the needs of population(s) served in a cost-effective manner.

  • Develops, provides, and evaluates educational programs to meet the identified needs of the population(s) served including patients, families, staff, and the community.

  • Monitors and ensures accuracy of documentation on patient records. Maintains all required reports, records, and statistics related to program.

  • Monitors the quality of care delivered to the population(s) served on a continual basis; identifies quality issues and integrates process improvement (P.I.) into practice.

  • May participate in research/investigative studies when indicated.

Minimum Requirements

  • Education - Associate degree in Nursing. Bachelor's degree in Nursing preferred.
