Role Description:

The Center Nurse is a clinical care team member operating within a Value-Based Care model, which prioritizes patient health outcomes and long-term wellness. In this model, the Center Nurse supports the health of patients by utilizing standards, guidelines and pathways for care delivery to prevent complications before they occur. Our Center Nurses drive quality care through clinical assessments, chronic condition management, and collaborating with the care team to reduce avoidable hospitalizations. You will foster an engaging and welcoming environment through effective team communication and delegation, empowering everyone on the team to deliver the best care to our patients.

Our Center Nurses report to the Practice Manager.

Core Responsibilities:

Support patient assessment and prioritization for both walk-in and telephonic triage to ensure patients receive the right level of care and avoid unnecessary hospital visits.
Execute protocol-driven chronic condition management.
Perform skilled nursing procedures including wound care and medication administration (including vaccines).
Provide comprehensive patient and caregiver education regarding chronic conditions and preventative care in person, over the phone, or in group settings.
Conduct medication reviews within nurse-led visits to ensure patient safety and adherence.
Conduct onsite testing (including point-of-care labs, EKGs, and specimen collection) and use patient data to identify and close gaps in preventative screenings, ensuring compliance with state scope of practice.
Review and act upon high-priority abnormal laboratory results in a timely manner to maintain patient safety and continuity of care.
Partner with providers and the interdisciplinary team to manage complex care plans and coordinate weekly transitions for patients moving from the hospital back to the clinic.
Utilize standardized protocols for medication management, prescription renewals and prior authorizations.
Ensures clinical communications are managed, tasks are completed and patient visit encounters are closed in a timely manner.
Delegates to medical assistants and phlebotomists as needed to support patient flow and efficiency.
Performs other related duties as assigned.

Provides emergency care in accordance with organizational procedure.

What we're looking for

Required Qualifications:

Active, Unrestricted Registered Nurse Licensure in the state that you reside
BLS or CPR Certification
Fluent written and spoken English language skills.
US work authorization
Electronic Medical Record (EMR) experience
Ability to maintain patient confidentiality and process information in a confidential manner
Ability to assess patients without face-to face interaction, strong communication and assessment skill

Strongly Preferred Qualifications:

Ability to collaborate and communicate with members of an interdisciplinary care team
Excellent computer skills with ability to read, interpret and analyze data from various computer systems
Effective problem solving and prioritization skills
2+ years of healthcare experience, working as an RN

The typical pay range for this role is:

Employment Type: Full time Shift: Day Shift Description: Registered Nurse (RNII) - Latham, NY - OBGYN - Full Time We are a busy and collaborative OB/GYN practice in Troy, New York seeking a Nurse to support our women’s health team with in-person clinical triaging. This role is stationed on site at our Troy office. The Nurse will be responsible for assessing patient concerns, responding to clinical phone calls and messages, coordinating care with providers, and supporting daily clinic flow. This position requires strong clinical judgment, excellent communication skills, and the ability to work efficiently in a fast-paced women’s health environment. The Registered Professional Nurse has the responsibility and accountability to utilize the nursing process to diagnose and treat human responses to actual or potential problems of individuals or groups. The Registered Professional Nurse works within and contributes to an environment where the St. Peter's Healthcare Services mission is actualized, patient outcomes are achieved, and professional practice is realized. Responsibilities: Brings patient to exam room, takes appropriate vital signs and documents in electronic medical record. Review and update medication list to ensure accurate and complete list in electronic medical record (EMR) available for provider review and submission. Complete referrals and tracks patients' compliance. Review prescriptions electronically and send prescriptions to providers for review and submission. Obtains patient consent for procedures as directed by provider. Performs pre-visit planning and reviews quality metrics. Retrieves telephonic clinical information from patients who call into the office. Monitors task list and completes tasks assigned by provider in a timely manner based on urgency. Educates patients regarding medication, testing procedures and home care techniques. Ensure proper labeling, handling and documentation for patient specimens. Follow up with patient regarding test results based on advice given by provider. Maintains a clean and safe work environment including disinfecting patient care areas and equipment. In conjunction with other nursing colleagues, maintains the medical supply cabinet and drug cabinet. Uses the electronic medical record to communicate effectively. Performs quality assurance duties as assigned. Provides a clinical visit summary (Patient Plan) to patient as requested including educational materials. Participates in daily Patient Care huddles as appropriate. Works cooperatively with all colleagues to ensure quality patient care at all times. Performs other duties as assigned. What you will need: Associates or Bachelor’s degree in Nursing preferred HS Diploma/equivalent required Current unencumbered NYS RN license Basic Life Support certification 6 months previous RN experience Must be able to lift 20 lbs. Pay Range: $36.00 - $47.52 Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
