About the Role:

As an Clinical Nurse Liaison at Aspire Senior Living, you will play a critical role in fostering effective communication between our nursing staff and medical professionals. Your expertise will help ensure seamless transitions, accurate documentation, and dignified care for our residents. Clinical Nurse Liaison will be responsible for:

• Coordinating with medical staff to facilitate smooth care transitions, including medication management, test results, and treatment plans
• Providing accurate and timely communication to residents, family members, and healthcare providers regarding resident care and status
• Collaborating with our nursing staff to develop and implement individualized care plans that meet the unique needs of each resident
• Maintaining accurate and up-to-date records, including medication administration records and progress notes

Requirements:

• Current LPN/RN licensure in the state of Missouri
• Minimum 1 year of experience in a long-term care setting
• Ability to work effectively in a fast-paced environment with multiple priorities
• Excellent communication, interpersonal, and problem-solving skills
