A Brief Overview
The Express Care clinics are a specialty within the nursing profession that encompasses the care of individuals across the life span who present with acute physical, emotional, or psychological alterations of health. The Express Care LPN responsibilities include, but are not limited to, patient assessment and triage, excellent communication and customer service skills, educating and answering patient questions during visit and at discharge. The Express Care LPN rooms patients, takes vital signs and reports patient concerns or pertinent observations to provider. The Express Care LPN is responsible for coordination of tests and services, assisting providers with procedures, and accurate documentation of all patient related activities. The Express Care LPN responds to emergent patient situations and provides patient care under the direction of the provider. The Express Care LPN may also perform procedures and testing under indirect physician supervision. Responsible to provide a safe environment for his/her assigned patients; to complete the assigned work; and to document and report any pertinent observations as a result of the interventions. The delivery of professional nursing care at Stormont Vail Health is guided by Jean Watson's Theory of Human Caring and the theory of Shared governance, both of which are congruent with the mission, vision, and values of the organization.

Licenses and Certifications

  • Licensed Practical Nurse - KSBN A multistate license with the ability to practice within the State of Kansas is also accepted. Required

What you will do

  • Assist with checking in, examinations and procedures.

  • Administer prescribed medications and treatments. Clarify medication orders and refills to pharmacies as directed by providers.

  • Triage patient telephone calls, evaluating the physical and psychosocial health status of the patient.

  • Coordinate patient services to include: lab tests, point of care testing, x-rays, diagnostic procedures, consults, home care, and acute care admissions.

  • Provide patient education regarding medications, lab tests, x-rays, and procedures. Notify patients of test results and provider disposition.

  • Document medical information using the appropriate electronic applications and/or forms.

  • Assess patient rooms and surrounding areas for safety issues. Remove hazards and make adjustments as indicated.

  • Utilize appropriate channels to communicate patient safety and patient care issues to appropriate parties.

  • Maintain cleanliness of equipment, examination, and treatment rooms.

  • Maintain exam rooms with necessary supplies and materials.

  • Participate in Patient Safety Rounds.

  • Adheres to infection control, emergency, safety, fire and disaster plans and policies, completing required initial training and updates per hospital policy.

What You'll Do As an LPN, you'll play an essential role in ensuring our residents receive safe, compassionate, and individualized care. As our Medical Records LPN, you'll play an important role in making sure our residents' clinical records are complete, accurate, confidential, and survey-ready while working closely with nursing, providers, and other members of our interdisciplinary team. What You'll Do Clinical Records & Documentation Maintain complete, accurate, current, and compliant clinical records for residents. Ensure documentation is appropriately dated, signed, legible, and securely maintained or scanned. Manage admission, transfer, discharge, and death records and required logs. Maintain the master patient census and resident master index. Help ensure clinical documentation remains organized, accessible, and survey-ready. Auditing & Compliance Conduct routine audits of clinical records, physician orders, MAR/TAR documentation, charting, and assessments. Monitor required admission, quarterly, annual, discharge, and physician documentation. Track physician visits, history and physical examinations, signed orders, and other required documentation. Complete required monthly and regulatory reports, including death reports and Ombudsman discharge reporting. Records Management Maintain secure systems for active and inactive clinical records. Track records maintained in off-site storage. Ensure records are retained, protected, and accessible in accordance with applicable requirements. Order and maintain clinical record forms and related supplies. Collaboration & Operational Support Partner with nurses, providers, leadership, and other departments to improve documentation accuracy and timeliness. Communicate audit findings and follow-up needs with the interdisciplinary team. Participate in care conferences, resident engagement rounds, and Manager of the Day responsibilities as assigned.
