We are seeking a compassionate and dedicated LPN Licensed Practical Nurse to join our team at Parkview Care Center, a leading provider of post-acute care and senior healthcare services in Fremont, Ohio. As an LPN, you will play a vital role in delivering high-quality care to our residents, providing emotional support, and creating a warm and welcoming environment.

About the Role

As an LPN at Parkview Care Center, you will be responsible for providing skilled nursing care to our residents, under the supervision of our Registered Nurses and interdisciplinary team. This includes assessing patient needs, developing and implementing care plans, administering medications, and providing wound care, among other responsibilities. You will work collaboratively with our staff to ensure that our residents receive comprehensive care that addresses their physical, emotional, and social needs.

Responsibilities

  • Provide direct patient care, including administering medications, wound care, and performing vital signs

  • Develop and implement individualized care plans in collaboration with the interdisciplinary team

  • Conduct initial and ongoing assessments of patients' physical and emotional needs

  • Implement interventions and treatments to address patients' care needs

  • Communicate effectively with patients, families, and healthcare team members

  • Maintain accurate and timely records of patient care and progress

Requirements

  • Current LPN license in the state of Ohio

  • High school diploma or equivalent required

  • Basic Life Support (BLS) certification required

  • Ability to work flexible hours, including evenings and weekends

  • Strong communication and interpersonal skills

  • Ability to work effectively as part of a team

  • Compassionate and patient-focused approach to care
