As a Registered Nurse (RN) - Hospice Palliative Care Lead, you will play a vital role in our organization's mission to provide exceptional care and comfort to our patients and their families. Located in Toledo, Ohio, our state-of-the-art facility is designed to provide a warm and welcoming environment for our patients to recover and heal.

About the Role:

  • Lead a team of Hospice Palliative Care RNs in providing compassionate and high-quality care to patients with life-limiting illnesses

  • Collaborate with interdisciplinary teams to develop and implement personalized care plans that focus on symptom management, pain control, and improvement of quality of life

  • Provide expert nursing assessment, intervention, and education to patients and their families

  • Manage care transitions and ensure continuity of care across settings

  • Conduct initial and ongoing assessments, identifying and addressing physical, emotional, and spiritual needs

Responsibilities:

  • Develop and maintain personalized care plans that meet the unique needs of each patient

  • Collaborate with interdisciplinary teams to develop and implement care plans

  • Provide expert nursing care and guidance to patients and families

  • Maintain accurate and timely records of patient care

  • Participate in quality improvement initiatives to improve patient outcomes and satisfaction

  • Act as a clinical resource and mentor to junior staff members

Requirements:

  • Current RN license in the state of Ohio

  • Bachelor's degree in Nursing (BSN) or related field

  • Minimum 2 years of experience in Hospice Palliative Care

  • Knowledge of current nursing standards, practices, and regulations

  • Strong communication and interpersonal skills

  • Ability to work effectively in a fast-paced environment
