About the Role:

As a Registered Nurse at Unity Health and Rehabilitation Center, you will be responsible for:

Providing high-quality patient care, including assessing, planning, implementing, and evaluating individualized care plans
Collaborating with interdisciplinary teams to develop and implement comprehensive care plans
Conducting regular patient assessments and reporting findings to the Medical Director and other healthcare professionals
Developing and implementing individualized education plans for patients and their families
Participating in quality assessment and improvement activities to ensure optimal patient outcomes
