We are seeking an experienced and detail-oriented MDS Coordinator to join our team. The ideal candidate will have a strong understanding of the MDS process, excellent organizational skills, and a commitment to providing quality, resident-centered care.

Position: MDS Coordinator
License: LVN or RN
Schedule: Full-Time
What You’ll Do
Coordinate and complete MDS assessments in accordance with current CMS, state, and regulatory requirements.
Ensure MDS assessments are accurate, timely, and supported by appropriate clinical documentation.
Coordinate the interdisciplinary team (IDT) assessment and care planning process.
Review clinical records, nursing documentation, therapy notes, physician documentation, and other relevant information.
Develop and update comprehensive care plans based on resident assessments and identified needs.
Collaborate closely with nursing, rehabilitation, dietary, social services, activities, physicians, and other members of the IDT.
Monitor MDS schedules and ensure assessments are completed within required timeframes.
Participate in resident care plan meetings and communicate effectively with residents and families.
Support accurate documentation and appropriate reimbursement related to Medicare/Medicaid services.
Assist with audits, surveys, and regulatory compliance activities.
Participate in QAPI initiatives and identify opportunities to improve clinical documentation and resident outcomes.
Provide education and guidance to staff regarding MDS requirements and documentation standards.
What We’re Looking For
Current, active LVN/LPN or RN license in the applicable state.
Previous experience in skilled nursing, long-term care, rehabilitation, or a related healthcare setting.
Previous MDS experience strongly preferred.
Knowledge of CMS/RAI requirements and long-term care regulations preferred.
Strong clinical assessment and documentation skills.
Excellent organization and attention to detail.
Ability to meet deadlines and manage multiple priorities.
Strong communication and teamwork skills.
