About Total Life

Total Life connects adults 65+ with licensed clinicians for behavioral health, geriatric care and longevity services, covered by Medicare, Railroad Medicare and major commercial insurers. Since 2021 we've grown to more than 200 credentialed providers serving patients in 49 states.

Most longevity medicine is built for healthy 40-year-olds. Ours is built for older adults. The Longevity Program combines a comprehensive geriatric assessment with chronic disease management, cognitive health, medication review and weight management, organized around five pillars of healthy aging: sleep, nutrition, movement, mental health and social connection. It works alongside a patient's existing care and never replaces their primary care provider.

The role

This is an in-person contract role. We're looking for nurse practitioners in the Tacoma-South Seattle area to lead longevity days at our partner senior living communities. Each day opens with a short educational talk for residents on healthy aging, using a presentation and speaker notes we provide. You then see the residents who sign up for one-on-one longevity visits.

You'll be trained and supported by our Chief Medical Officer, a board-certified geriatrician who designed the program's clinical content. Your first site day is run alongside a member of our team.

Schedule

Site days are on weekdays. The presentation is the fixed part of the day. The rest depends on how many residents sign up. Expect roughly 4 to 8 hours per site day, a few times a month to start, with hours growing as bookings grow.

What you'll do
  • Conduct 45 to 60 minute longevity visits built on a comprehensive geriatric assessment: what matters most to the resident, medication review and deprescribing, cognition (including the Mini-Cog), mood, sleep, nutrition, mobility, fall risk, daily function and advance care planning
  • Evaluate eligible residents for weight management, including GLP-1 therapy where clinically appropriate
  • Identify residents who would benefit from follow-up care, such as a fuller cognitive review, ongoing medical follow-up, psychiatry or therapy, and coordinate with their primary care provider
  • Document every encounter accurately and completely, to Medicare standards, in our EHR
  • Represent Total Life professionally with residents, families and community staff
  • Travel to assigned senior living communities in the Tacoma-South Seattle area
Requirements
  • Active, unrestricted Washington ARNP license as a nurse practitioner, with prescriptive authority, from the Washington State Board of Nursing, plus an active Washington or multistate RN license (a multistate RN compact license does not cover ARNP practice)
  • Current national NP board certification
  • Active DEA registration and registration with the Washington Prescription Monitoring Program
  • Clinical experience caring for adults 65+
  • Experience with Medicare documentation and E/M coding
  • Your own malpractice insurance (minimum $1M per occurrence, $3M aggregate) that covers in-person care, not just telehealth, or the ability to obtain it before your first site day
  • Comfort presenting to groups and speaking with older adults and their families
  • A valid driver's license and reliable transportation
  • Ability to complete each community's onboarding requirements, which may include a background check, TB screening and immunizations
  • Weekday availability for on-site days in the Tacoma-South Seattle area, with hours confirmed in advance
Nice to have
  • Experience with comprehensive geriatric assessment
  • Familiarity with cognitive screening and staging tools (Mini-Cog, MoCA, FAST); MoCA certification is a plus
  • Experience in senior living, long-term care, SNF, hospice, palliative or house-call settings
  • Experience prescribing GLP-1s for weight management
  • Telehealth experience
  • Licenses in more than one state
  • Adult-Gerontology NP certification (AGNP/AGPCNP), or Family NP with a geriatric focus or training

The ARNP/PA-C is a certified physician assistant or advanced registered nurse practitioner who provides evaluation and medical management to Heme/Heme Malignancy patients at Fred Hutch Cancer Center. Advanced Practice Provider: Undiagnosed Mass Clinic

The Undiagnosed Mass Clinic (UDM) PA-C/ARNP is a physician assistant or advanced registered nurse practitioner who provides dedicated support to this growing Fred Hutch program, which serves as a diagnostic entry point for patients with suspected cancer. The APP provides comprehensive evaluation, diagnostic workup coordination, and medical management for patients referred to suspected malignancy, unexplained mass, or abnormal diagnostic finding requiring further oncologic assessment. The APP independently evaluates new referrals, develops diagnostic plans, orders and interprets testing within scope of practice, coordinates care across specialties, communicates findings to patients and referring providers, and facilitates timely transition to disease-specific oncology teams when cancer is confirmed. When cancer is not identified, the APP collaborates with referring providers and primary care teams to ensure appropriate follow-up and disposition. The UDM APP can practice at the intersection of oncology, diagnostic medicine, and care coordination. Significant collaboration with nurse navigation, pathology, radiology, and disease-specific oncology teams is necessary. All clinical responsibilities are performed within the APP scope of practice, institutional privileges, and under the supervision of an attending physician. This is a part time (50% FTE) position. Responsibilities Perform comprehensive and problem-focused H&P for patients referred to the UDM Clinic. Review referral records, imaging, pathology, lab studies, and prior evaluations. Develop, order, and manage diagnostic workups for patients with suspected malignancy. Interpret and communicate diagnostic findings to patients, referring providers, and consulting teams. Coordinate diagnostic testing, specialty consultations, and follow-up evaluations across multiple disciplines and healthcare settings. Collaborate closely with wide range of team members and specialists to ensure efficient progression through the diagnostic process. Track and manage pending studies, referrals, pathology results, and other diagnostic milestones to ensure timely completion of evaluations. Provide follow-up care and medical management while diagnostic workup is underway. Facilitate hand-offs and referrals to disease-specific oncology services when cancer is diagnosed. Coordinate care transitions back to primary care providers, referring physicians, or other specialty services when malignancy is excluded. Provide patient and caregiver education regarding diagnostic plans, test results, treatment recommendations, and follow-up care. Serve as a clinical resource for nurse navigation and intake referral processes. Participate in multidisciplinary conferences, QI initiatives, and departmental committees as assigned. Communicate effectively with inpatient and outpatient teams to support continuity of care. Participate in program development efforts that support growth, operational efficiency, and access within the UDM Clinic. Additional duties as assigned. Qualifications Required: Graduate of an accredited Physician Assistant program or Nurse Practitioner program. Current Washington State licensure as a Physician Assistant-Certified (PA-C) or Advanced Registered Nurse Practitioner (ARNP), or ability to obtain licensure prior to employment. DEA registration required upon credentialing. Minimum of four years of APP experience in oncology, hematology, surgical oncology, hospital medicine, internal medicine, or other relevant specialty. Demonstrated clinical expertise in complex patient evaluation and diagnostic reasoning. Strong collaborator with the ability to work effectively independently and with a multidisciplinary team. Excellent verbal and written communication skills. Experience utilizing electronic medical record (EMR). Strong organizational skills and ability to manage multiple patient workups simultaneously. Ability to work independently while maintaining collaboration with supervising physicians and teams. Preferred: Broad experience in oncology, cancer diagnostics, hematology, and complex medical evaluation. Experience coordinating multidisciplinary care across healthcare systems. Experience with quality improvement and program development initiatives.
