The Home Health & Hospice RN (HHHRN) provides care to patients who are in need of care in their homes. Home health & hospice nursing practice considers the needs of the patient, their family and/or the patient’s caregivers. The HHHRN provides care to patients across the lifespan, from prenatal to post-death periods. Home health nursing takes a holistic approach to personal health practices when treating diseases or disabilities. The practice embraces primary, secondary and tertiary prevention; assisting patients and families to coordinate community resources and health insurance benefits as well as hands on delivery of nursing services in the patient’s home.

I. Major Responsibilities:
1. Assures case coordination and provides direct nursing care to patients and families on a 7 day per week (Sunday to Saturday) basis including day, evening, night, weekend, and holiday duty.
2. Meets patient care productivity standards established by the agency.
3. Collects comprehensive data pertinent to the patient’s health or the situation.
4. Analyzes assessment data to determine the diagnoses or issues.
5. Identifies expected outcomes in a plan individualized to the patient, family and/or caregivers and the situation.
6. Develops a plan that prescribes strategies and alternatives to attain expected outcomes.
7. Implements the individualized patient plan of care.
8. Coordinates care delivery.
9. Employs strategies to promote health, comfort, and a safe environment.
10. Evaluates progress towards attainment of outcomes.
11. Systematically enhances the quality and effectiveness of his/her nursing practice.
12. Attains knowledge and competency that reflects current nursing practice.
13. Evaluates one’s own nursing practice in relation to professional practice standards and guidelines, relevant statutes, rules, and regulations.
14. Interacts with peers and colleagues, and contributes to their professional development.

II. Position Qualifications:

License/Certification/Education:
Required:
1. Diploma or Associate’s Degree in Nursing.
2. Valid and unrestricted licensure as a registered nurse in the Commonwealth of Massachusetts.
3. Valid and unrestricted driver’s license recognized by the Commonwealth of Massachusetts and access to a reliable, insured vehicle.

Preferred:
1. Bachelor’s, Master’s, or Doctorate in Nursing. 
2. Specialty nursing certification as a home health nurse, hospice and palliative care nurse or case manager. 
3. Completion of the Certified Oasis Specialist certificate.

Experience/Skills:
Required:
1. Ability to perform all duties according to American Nurses Association Scope & Standards of Practice for Home Health Nursing professional standards and HAHHH policies and procedures.
2. Ability to use a laptop computer.  
3. Ability to proficiently utilize or learn to proficiently utilize MS Office applications, e-mail, Point-of-Care software applications, and other wireless applications.
4. Ability to perform comprehensive physical and psychosocial assessments.
5. Ability to work autonomously.
6. Ability to communicate professionally and effectively with a diverse patient population.
7. Ability to read, write and speak clearly in English.

Preferred:
1. 2-5 years of recent (within the past 18 months) medical-surgical experience either in an acute or subacute setting or same experience working in a certified home health agency or licensed hospice.

2. Previous case management or program management experience. 

Unless certification, licensure or registration is required, an equivalent combination of education and experience which provides proficiency in the areas of responsibility listed in this description may be substituted for the above requirements.

Provides ongoing interdisciplinary assessment of the patient. 2. Assesses patient/family learning needs, style and limitations and makes adjustments for delivery of information. 3. Recognizes/identifies patients/ caregivers’ cultural beliefs re: death and dying and is sensitive to those beliefs. 4. Establishes realistic goals and develops plan of treatment in cooperation with the patient, family and members of the health care team. 5. Identifies abuse and/or neglect and identifies reporting mechanism per policy. 6. Adapts to new and unusual situations without affecting work performance negatively. Utilizes Security when and if needed for any potential unsafe situations. 7. Collaborates with patient /family and other health care providers and/or community resources with planning of care and discharge. 8. Continuously assesses/evaluates patient and family progress and goal achievement, keeping Clinical Manager advised and justifying continuation and/or termination of services. 9. Documents patient data, activities, and communications in patient record using computer system in a timely , organized, concise manner to ensure compliance with legal, certification, third party payor guidelines and Agency policies. 10. Completes physicians' orders, levels of care, and OASIS on all patients assigned, in accordance with patient care policies. Reconciles medications with patient and physician consistently. 11. Demonstrates and practices according to safety guidelines and procedures including general ergonomics, fire, and personal safety . Utilizes proper body mechanics and patient material handling. 12. Demonstrates an awareness of safety and security issues by responding appropriately to emergency situations and following established procedures and protocols utilizing stabilizing techniques. 13. Demonstrates ability to cope with patient/family emotional stress and provide appropriate supportive care. 14. Effectively manages assigned caseload, within the team model of care delivery . Establishes a daily work plan based upon patient/family priorities of service and total area needs. Promptly triages patient visits, messages, and phone calls according to priority and urgency . 15. Provides nursing coverage on a rotating basis for evenings, nights, weekends and Holidays. 16. Demonstrates excellent physical assessment and care planning skills. Demonstrates current knowledge of pharmacology and medication administration and reconciliation. 17. Coordinates care and discharge planning with other team members during case conferences. 18. Acquires and maintains an expert working knowledge of all third part payers and regulatory bodies and agency policies on issues related to documentation and care provided. 19. Communicates and collaborates with all disciplines in the home care setting on a regular basis or immediately if there are any critical needs or crisis interventions needed.
