Role Details:

  • Provide 1:1 nursing care to a student as needed within a school environment
  • Part-time aid coverage needed October–December: 1-2 days/week from 8am–12:15pm
  • January onward: coverage needed 5 days/week from 8am–12:15pm
  • Collaborate with educational staff to support student’s day-to-day health needs
  • Assess and document ongoing medical needs
  • Assist with the development of medical care plans as hours expand

Qualifications:

  • Active RN license in Ohio
  • Prior experience with school nursing or pediatric care preferred
  • Ability to work independently and in coordination with educational teams
  • Strong communication and organizational skills
  • Flexibility to adapt as hours and needs expand

This role is responsible for building strong community relationships, identifying hospice-appropriate patients, coordinating admissions, and ensuring a compassionate, seamless transition into hospice services. Key Responsibilities Referral & Relationship Management Build and maintain strong relationships with hospitals, skilled nursing facilities, physicians, social workers, and other referral partners. Respond promptly to new referrals and inquiries, ensuring timely follow-up and exceptional customer service.
