Job Summary:

As a Home Care Aide, you will provide personalized care and support to our clients in their own homes. Your responsibilities will include:

Assisting with daily living activities such as bathing, dressing, and grooming
Providing companionship and social interaction to help reduce feelings of loneliness and isolation
Supporting with light housekeeping, meal preparation, and medication reminders
Collaborating with our clients' families and healthcare providers to ensure seamless care

What You'll Need:

High school diploma or equivalent required
Registered as a Home Care Aide (or willing to register) CNA is also preferred
Reliable transportation and a valid driver's license
Ability to read, write, and speak English as it pertains to the job
Compassion, empathy, and a genuine desire to help others

Garfield Ave. 2nd Floor, Alhambra, CA 91801 Compensation: $27.00 - $35.00 / hour Description We are seeking a compassionate, dedicated, and detail-oriented Certified Home Health Aide (CHHA) to join our hospice interdisciplinary team. This field-based role focuses on providing essential personal care and emotional support to terminally ill patients, ensuring comfort, dignity, and quality of life across the San Gabriel Valley (SGV) region. The ideal candidate will work independently in the field, confidently manage digital documentation, and possess a deep respect for diverse cultural backgrounds. Our Values: Put Patients First Empower Entrepreneurial Provider and Care Teams Operate with Integrity & Excellence Be Innovative Work As One Team What You'll Do Assist patients with activities of daily living (ADLs) including bathing, dressing, grooming, feeding, and oral hygiene under the direction of a Registered Nurse/DPCS. Provide skin care to prevent breakdown, assist with safe ambulation, transfers, and prescribed exercises, ensuring a safe and supportive environment. Monitor and record vital signs as directed. Document and immediately report any changes in the patient’s physical, mental, or emotional condition to the Case Manager or Supervisor. Use the HospiceMD EMR platform to log patient visits, care plans, and clinical narratives accurately and timely, in accordance with agency policy and regulatory standards. Travel reliably between assigned patient locations across the SGV territory. Track business travel and accurately submit monthly mileage invoices using Concur for reimbursement. Work collaboratively with nurses, social workers, chaplains, and families to carry out the comprehensive plan of care. Provide emotional support and companionship to patients and their loved ones. Additional duties as assigned. Qualifications Active California Certified Home Health Aide (CHHA) license in good standing. Current and valid CPR certification. Proficiency in Mandarin or Cantonese is strongly preferred to best serve our diverse patient demographics in the SGV area, though it is not strictly required. Fluent verbal and written English communication is required. Strong computer knowledge with previous hands-on experience using Electronic Medical Records (EMR) software, specifically HospiceMD, and comfort with digital expense tools like Concur. Valid driver’s license, a reliable and well-maintained vehicle, and active auto liability insurance to meet mandatory field travel requirements. Deep empathy for the ill and elderly, a calm demeanor around end-of-life care, high integrity, and strict adherence to HIPAA patient confidentiality regulations. Lifting & Mobility: Ability to lift, push, pull, or move up to 50 pounds frequently. Ability to safely stand, walk, bend, stoop, kneel, and assist patients with varying physical mobility levels throughout an 8-hour field shift. The national target pay range for this role is $27.00 - 36.00 per hour. Actual compensation will be determined based on geographic location (current or future), experience, and other job-related factors.
