As the Hospice Nurse Practitioner, you will provide consultation in palliative care, symptom management and supportive care to meet the needs of the Hospice patient as requested by the referring physician. 

 

Primary Responsibilities:

  • Makes face-to-face encounter visits within prescribed time limits to qualifying Hospice patients and reports clinical findings to the Hospice Medical Director  
  • Consults with the Hospice Medical Director as needed, informs primary physician of recommended services, and collaborates with other physicians as needed
  • Initiates and coordinates the plan of care. Documents problems, appropriate goals, interventions, and patient/family response to hospice care. Collaborates with the patient/family, attending physician and other members of the IDG in providing patient and family care
  • Instructs and supervises the patient/family in self-care techniques when appropriate
  • Maintains accurate and relevant clinical notes regarding the patient's condition. Reports patient condition and any identified needs/issues to Administrator and Hospice Medical Director

Required Qualifications:

  • Active unrestricted license in the state of residence
  • Current certification as a clinical Nurse Specialist or Nurse Practitioner in the state of practice
  • Current certification by a national body such as ANCC, NBCHPN, or AANP
  • 5+ years nursing experience  
  • Current driver's license and vehicle insurance, access to a dependable vehicle or public transportation

 

Preferred Qualifications:

  • Current CPR certification or ability to complete within 90 days of hire
  • Experience working with an interdisciplinary team
  • Clinical nursing care in oncology, hospice, geriatrics, or intensive care.
  • Palliative Care ARNP

 

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). About Total Life Total Life connects adults 65+ with licensed clinicians for behavioral health, geriatric care and longevity services, covered by Medicare, Railroad Medicare and major commercial insurers. Since 2021 we've grown to more than 200 credentialed providers serving patients in 49 states. Most longevity medicine is built for healthy 40-year-olds. Ours is built for older adults. The Longevity Program combines a comprehensive geriatric assessment with chronic disease management, cognitive health, medication review and weight management, organized around five pillars of healthy aging: sleep, nutrition, movement, mental health and social connection. It works alongside a patient's existing care and never replaces their primary care provider. The role This is an in-person contract role. We're looking for nurse practitioners in the West Valley (Phoenix) area to lead longevity days at our partner senior living communities. Each day opens with a short educational talk for residents on healthy aging, using a presentation and speaker notes we provide. You then see the residents who sign up for one-on-one longevity visits. You'll be trained and supported by our Chief Medical Officer, a board-certified geriatrician who designed the program's clinical content. Your first site day is run alongside a member of our team. Schedule Site days are on weekdays. The presentation is the fixed part of the day. The rest depends on how many residents sign up. Expect roughly 4 to 8 hours per site day, a few times a month to start, with hours growing as bookings grow.
