Job Summary:

We are seeking an experienced, compassionate, and forward-thinking Director of Nursing to lead our clinical care team and help shape the future of in-home care in our community. About the Role:

The Director of Nursing (DON) is responsible for overseeing all clinical and care management operations for our home care agency. This position ensures that our care is safe, high-quality, compliant, and aligned with SYNERGY HomeCare's mission to provide personalized, person-centered support that helps clients remain independent at home.

Responsibilities:

  • Provide clinical leadership, direction, and oversight for all client care services

  • Conduct and/or oversee client assessments, develop individualized care plans, and ensure accurate, timely documentation

  • Supervise, mentor, and support our team of nurses aides and caregivers, fostering a culture of respect, accountability, and continuous improvement

  • Monitor quality of care, conduct chart reviews, and implement quality assurance and performance improvement initiatives

  • Ensure compliance with all applicable federal, state, and local regulations, as well as company policies and professional standards

  • Serve as a key clinical resource and liaison for clients, families, physicians, referral partners, and internal staff

  • Participate in on-call rotation and support clinical problem-solving as needed

What We’re Looking For:

  • Current, active RN license in good standing in the state of practice

  • Bachelor’s degree in nursing (BSN) preferred; advanced education or certifications a plus

  • 3–5+ years of clinical nursing experience, preferably in home care, home health, hospice, or related community-based setting

  • Prior leadership or supervisory experience (e.g., charge nurse, nurse manager, DON, clinical supervisor)

  • Strong knowledge of care planning, documentation standards, and regulatory requirements for home-based care

  • Excellent communication, critical thinking, and problem-solving skills

  • Demonstrated ability to build, develop, and retain high-performing clinical teams

  • Genuine commitment to person-centered, compassionate care and to serving older adults and individuals with chronic or complex needs

Under general supervision, provides professional nursing care for patients adhering to national and company standards and guidelines for specialty regulations. Must recognize physical, psychological, and spiritual aspects of care and participates in company wide quality initiatives. Supports and adheres to the US Oncology Compliance Program, to include the Code of Ethics and Business Standards. Responsibilities ESSENTIAL DUTIES AND RESPONSIBILITIES: -Demonstrates knowledge of age specific differences in the patient population including physiological and developmental differences unique to each group. -Assess and monitors therapy of patients using appropriate technical skills under the direction and supervision of a physician. -Assists physician with exams and procedures. -Maintains efficient and appropriate patient flow. -Demonstrates knowledge of reimbursement procedures and charge capture. -Performs venipuncture, accesses implanted ports and maintains central venous access devices and alternate delivering systems. -Documents all elements of care. -Communicates all pertinent information and any interventions to the physician or mid level provider. -Serves as a resource person to the medical assistants and LPNs. -Triages patient/family phone calls as needed and documents relevant information in the patient's chart. -Assesses patient/family needs and provides teaching. -Demonstrates awareness of community resources available to patients and assists with referrals if necessary. -Provides nursing coverage for the satellite offices as needed. -Follows OSHA guidelines when handling cytotoxic agents, biohazardous waste and any other material potentially infected with a blood borne pathogen. -Maintains current knowledge of oncology nursing by participating in ongoing educational opportunities. -Participates in professional development activities and maintain professional affiliations. -Maintains patient confidentiality. -Serves as mentor/preceptor and provides day to day guidance to less experienced nurses.
