Responsibilities

1. Facilitates the RAI Process under the direction of an R.N. or DNS2. Work in Collaboration with the interdisciplinary team to identify the needs of the patient/resident.3. Coordinates and conducts the daily Case Management meeting.4. Ensures timely MDS assessments according to state and federal regulations.5. Ensures interdisciplinary team completes designated sections of the MDS assessment.6. Ensures and completes accurate coding of the MDS assessment with information obtained via medical record review, observation and interview with center staff, patients, residents and family members.7. Ensures documentation is available to support accurate coding of the MDS assessment.8. Maintains the tracking system of MDS assessment schedules (timeframes and due dates).9. Facilitates Care Plan conferences with the interdisciplinary team, patient, residents and families.10. Obtain, review and maintain all State and Federal reports, making appropriate correctionstimely.11. Monitors Quality Measures and ensures that MDSs are accurate to support and reflect theQuality Measures.12. Provides education related to the RAI Process13. Coordinates and completes electronic submission of required documentation to the Statedata base and other entities per company guidelines and State and Federal regulations.14. Ensures Medicare and Medicaid regulatory guidelines are completed accurately and timely(i.e.: certifications, denial letters, skilled documentation, coverage criteria, etc…)15. Provides Managed Care case management at the center level.16. Reviews Additional Documentation Requests (ADRs) with Health InformationManagement Coordinator (HIMC) to ensure all documents are available and meet therequest.17. Participates in billing reviews (Triple Check) to ensure claims have supporting MDSassessments documents.18. Participates in Quality Assurance and Process Improvement Activities.19. Continues to update knowledge base related to Medicare, Managed Care, Medicaid, RAIProcess and computer technology updates.20. May be required to fulfill the job functions of a direct care nurse as determined by the needs ofthe patients/residents and center

Qualifications

1. Must hold current nursing license in the state of employment.2. Experience in Long Term Care.3. Computer skills.4. Knowledge of Medicare, Managed care and RAI Process Required.5. Participating in the LPN to RN Back to School transition program.

Registered Nurse RN PreOp/PACU - Full Time

The Pre-Operative/Post-Operative Registered Nurse, under the supervision of the PACU Manager or the Director of Nursing, according to the organizational structure, is responsible for providing peri-operative patient care in accordance with sound nursing judgment, regulatory and accreditation standards, and the facility's mission, vision, and values. The position must provide direct and indirect patient care across the age continuum; and utilize the nursing process of assessment, planning, implementation, and evaluation according to policies and procedures of the facility. Assesses and documents patient's physical and emotional status upon arrival in the PACU, provides nursing care to meet patient needs related to surgical condition and type of anesthesia administered, performs safe and individualized nursing care for the patient in the immediate postoperative period, develops a plan of care based on the assessment data, monitors patients and informs surgeon or anesthesia of patient status and notifies of changes/complications in a timely manner, provides comfort and reassurance to the patient and promotes privacy and dignity, reviews postoperative instructions from the surgeon with patient and caregiver and documents in medical record, provides written discharge instructions and any needed supplies, calls patients postoperatively to assess postop condition. Essential Job Functions: Shows appropriate delegation, while directing and supervising ancillary personnel in planning, implementing, monitoring and evaluating patient care in the pre-operative/post-operative clinical areas according to accepted standards. Provides direct patient care in the assigned clinical areas and demonstrates clinical competence. Participates in service recovery by taking ownership of the situation and correcting problems. Keeps customers informed of progress related to clinical operations and provides comfort, information and assistance during the process.
