As a compassionate and dedicated Licensed Practical Nurse (LPN), you will play a vital role in delivering exceptional care to our residents.

Job Summary:

The Licensed Practical Nurse (LPN) provides direct nursing care to residents in accordance with physician orders, facility policies, state and federal regulations, and accepted standards of nursing practice. The LPN works under the supervision of a Registered Nurse and collaborates with the interdisciplinary team to ensure quality resident care and services.

Responsibilities:

  • Administer medications and treatments as prescribed by physicians.

  • Monitor residents' health status and report significant changes to the physician and supervising nurse.

  • Provide wound care and other clinical interventions

  • Perform nursing assessments within scope of practice.

  • Ensure accurate and timely documentation is informative and in a descriptive manner.

  • Admit, discharge, and transfer residents.

  • Supervise CNAs to ensure adherence to care standards in accordance with current rules, regulations, and guidelines

  • Maintain infection prevention and control practices

  • Support implementation and updates of individualized care plans for each resident.

  • Safeguard resident rights, privacy, and dignity (HIPAA compliance)

  • Notify the residents' attending physicians and next-of-kin when there is a change in the residents' conditions or if the residents are involved in an incident or accident

  • Attend and participate in all center in-service programs as required

Qualifications:

  • Must hold a current and valid LPN license in the state employed

  • Long-term care/post-acute care setting preferred

  • Completion of post-secondary education

  • Capable of maintaining regular attendance (i.e., weekend shifts and overtime) as required
