Provides supervision/coordination of all nursing care given by each member of nursing staff and teaches nursing care when indicated. Supervises, coordinates, and gives instructions on all written and verbal recordings, including resident’s records and any other forms or records kept by ward personnel. Administers medications and treatments as indicated and carries out associated recording procedures. Initiates resident care plans, draws blood samples for lab work, administers oxygen, uses suction machine, and gives CPR when indicated. Performs other life-saving measures as needed. Accompanies physicians on their visits to the residents, assisting with examinations and diagnostic measures when indicated. Make daily rounds, observing all residents, assessing their health status and determining any special needs. Notifies physician of any changes in the mental and physical condition of residents. Participates in admitting, transferring, and discharging of residents, ensuring that clothing and valuables are taken care of. Writes and checks all records and reports and instructs ward personnel in these. Maintains records and communications as necessary for effective total resident care. Responsible for supervision/coordination of ward staff in proper care of residents, and assures that nursing care and resident responses to care are documented. Participates in Service Team meetings. Formulates and reviews care plans as indicated. Delivers and receives report about residents and nursing care. Inspects the ward for cleanliness, ventilation, temperature, and safety measures. Provides instructions to ward personnel in use of equipment to prevent damage to equipment and waste of supplies. Confers with other disciplines and departments about supplies, transportation for residents, repairs and general upkeep of the ward. Checks supplies in cabinets, treatment rooms, dressing carts, and medicine carts for expired items and general cleanliness. Checks oxygen tanks, emergency cart, and suction machines for proper function. Conducts regular ward meetings. Keeps Nursing Supervisor and Executive Nursing Director informed about ward activities and problems. Attends inservice and unit meetings as required. Maintains compliance with unit and FSH mandatory inservice training. Performs other related duties. Initiates and assists with emergency measures as directed by RN in charge. Has a thorough understanding of all equipment on acute care units, including but not limited to; beds, scales, IV pumps, enteral feeding pumps, SCDs, respiratory equipment, etc. Can initiate and maintain a sterile field and assist the physician as necessary with suturing, chest tube insertion, lumbar puncture, etc. Maintains current knowledge of medications and their correct administration based on age specific criteria, patient clinical condition and safety policy. Ensures that nursing performs daily/nightly narcotic medication reconciliation checks with another licensed nurse to verify correct administration/documentation of scheduled drugs. Performs AccuChek testing per policy, ensures that controls are dated with opened and expiration date; responds and documents any actions taken in response to results. Labels all medications, medication containers (e.g., syringes, medicine cups, basins) or other solutions on and off the sterile field in procedural settings. Compiles a complete list of the patient’s medications to assist in reconciliation of medications across the continuum of care. Prioritizes activities for proper balance of patient care activities/assignments within assigned shift. Maintains and updates patient charts per unit policy. Checks each chart for completeness prior to releasing record to health information. Provides complete documentation on nurse’s notes, flow sheets and graphic sheets. Transcribes physician orders for medications to the MAR correctly; charting correct medication, dose, route, frequency and timing all doses to be given correctly. Maintains current knowledge of medical staff approved hospital pharmacy formulary and uses correct substitutions when indicated. Provides a safe environment for patients and family members. Reduce the risk of patient falls by keeping patient rooms and areas free of excess clutter, removes trash at the end of shift Complies with infection control standards, performs personal safety precautions (PPE) appropriately. Takes professional responsibility to wear personal protective equipment based on potential risk exposure. Follows infection control standards by washing hands or using alcohol gel between all patients and before/after performing any procedures. Provides appropriate discharge instructions to patients and family members in a timely manner. Includes instructions and referrals based on patient/family needs and physician orders. Notifies physicians or caretakers of all critical test or lab results in a timely manner and verifies understanding by having physician repeat values back as same. Documents the communication and information reported in patient record. Gives accurate and complete patient report to any nurse assuming responsibility for any patient. Ensure shift duties are completed to include stocking, narcotic count, crash cart and ensuring that refrigerator temperatures are checked on a daily basis. Assists new personnel orient to department, reviews and completes skills assessments for new personnel, and reports educational needs and concerns to nurse manager. Assists other staff members keep work area clean and tidy. Uses break room for all meals and snacks. Removes personal items from work area when shift complete.
