Ridge Valley Center for Nursing is seeking a compassionate and experienced Registered Nurse (RN) to join our team for our 7 PM–7 AM overnight shift.

What You'll Do

  • Provide compassionate, resident-centered nursing care throughout the overnight shift.

  • Assess residents' conditions and monitor for changes in health status.

  • Develop, implement, and evaluate individualized care plans in collaboration with the healthcare team.

  • Administer medications and treatments safely and accurately according to physician orders and facility policies.

  • Monitor residents and respond promptly to changes in condition or adverse symptoms.

  • Perform treatments, wound care, IV therapy, catheter care, and other nursing procedures as needed.

  • Accurately document assessments, medications, treatments, vital signs, and other clinical information.

  • Communicate effectively with physicians, residents, families, and members of the interdisciplinary team.

  • Participate in care planning and discharge planning as appropriate.

  • Promote resident safety, dignity, comfort, and quality of life.

  • Follow all facility policies, procedures, and applicable regulations.

What We're Looking For

  • Current, active Registered Nurse (RN) license in North Carolina.

  • Current CPR certification.

  • Strong clinical assessment and critical-thinking skills.

  • Excellent communication and documentation skills.

  • Ability to work independently while collaborating effectively with the nursing team.

  • Previous skilled nursing or long-term care experience is preferred.

RN Wound Care Nurse

Ridge Valley Center for Nursing & Rehab is seeking a Wound Care Nurse. Summary: As the Wound Care Nurse, you will provide primary skin care to guests, with an emphasis on treatment and therapy of skin disorders. You will also assist in modifying the treatment regimen to meet the physical and psychosocial needs of the guest, in accordance with established medical practices and the requirements of this state and the policies and goals of this facility . Environment: Work will be performed primarily indoors at one of our long-term healthcare facilities, throughout all areas, including in resident rooms, and on carpeted and/or tiled floors. Work will be performed there routinely around other co-workers, healthcare staff, residents, and guests. Due to the nature of facility’s business, worker may be exposed to occasional slippery floors, object on floors, chemicals, sharp objects, hazardous materials and waste (including human), blood borne pathogens, and communicable diseases, as well as high-stress medical and/or life-threatening situations. Essential Duties & Responsibilities: Meet physical and sensory requirements stated below and be able to work in the described environment. Identify and participate in process improvement initiatives that improve the customer experience, enhance workflow , and/or improve the work environment. Direct the day-to-day functions of the nursing assistants in accordance with rules, regulations, and guidelines that govern the long-term care industry. Confers with the Director of Nursing and/or other licensed nursing personnel regarding dermatologic disorders of guests in the facility. Identifies , manages, and treats specific skin disorders, pressure ulcers, primary and secondary lesions, and any skin issues directed to by Director of Nursing. Consults with the IDT concerning assessment evaluations and assist in planning and developing the skin Implements and maintains established policies and procedures relative to skin care treatments and interprets these as required , to the physician and responsible party. Provides, within established protocols and under the physician's supervision, treatment for skin disorders as ordered by the physician. Ensures that residents with decubitus ulcers, vascular ulcers, and various other skin conditions receive appropriate treatment and/ or prophylactic treatment such as daily inspection, turning and activity, nourishment, and incontinence care Participate in surveys (inspections) made by authorized government agencies. Periodically review the resident’s written discharge plan. Participate in the updating of the resident’s written discharge plan, as required. Assist in planning the nursing portion of the resident’s discharge plan, as necessary. Admit, transfer, and discharge residents, as required. Complete accident/incident reports, as necessary. Complete and file required forms/charts upon resident’s admission, transfer, and/or discharge. Encourage attending physicians to review treatment plans, record and sign their orders, progress notes, etc., in accordance with established policies. Receive telephone orders from physicians and record on the Physicians’ Order Form. Transcribe physician’s orders to resident charts, cardex , medication cards, and treatment/care plans, as required. Chart nurses’ notes in an informative and descriptive manner that reflects the care provided to the resident, as well as the resident’s response to care. Record new/changed diet orders. Forward information to the Dietary Department. Report all discrepancies noted concerning physician’s orders, diet change, charting error, etc., to the Nurse Supervisor, Unit Manager, or ADON/DON. Fill out and complete transfer forms in accordance with established procedures.
