We are seeking an experienced and detail-oriented MDS Coordinator to join our team. The ideal candidate will have a strong understanding of the MDS process, excellent organizational skills, and a commitment to providing quality, resident-centered care.

Position: MDS Coordinator

License: LVN or RN
Schedule: Full-Time

What You’ll Do

  • Coordinate and complete MDS assessments in accordance with current CMS, state, and regulatory requirements.

  • Ensure MDS assessments are accurate, timely, and supported by appropriate clinical documentation.

  • Coordinate the interdisciplinary team (IDT) assessment and care planning process.

  • Review clinical records, nursing documentation, therapy notes, physician documentation, and other relevant information.

  • Develop and update comprehensive care plans based on resident assessments and identified needs.

  • Collaborate closely with nursing, rehabilitation, dietary, social services, activities, physicians, and other members of the IDT.

  • Monitor MDS schedules and ensure assessments are completed within required timeframes.

  • Participate in resident care plan meetings and communicate effectively with residents and families.

  • Support accurate documentation and appropriate reimbursement related to Medicare/Medicaid services.

  • Assist with audits, surveys, and regulatory compliance activities.

  • Participate in QAPI initiatives and identify opportunities to improve clinical documentation and resident outcomes.

  • Provide education and guidance to staff regarding MDS requirements and documentation standards.

What We’re Looking For

  • Current, active LVN/LPN or RN license in the applicable state.

  • Previous experience in skilled nursing, long-term care, rehabilitation, or a related healthcare setting.

  • Previous MDS experience strongly preferred.

  • Knowledge of CMS/RAI requirements and long-term care regulations preferred.

  • Strong clinical assessment and documentation skills.

  • Excellent organization and attention to detail.

  • Ability to meet deadlines and manage multiple priorities.

  • Strong communication and teamwork skills.

  • Commitment to quality care and resident-centered service.
