Frederick Health Medical Group (FHMG), a highly respected 160+ provider, multi-specialty medical group, is seeking an experienced and motivated Advanced Practice Provider (APP) - Nurse Practitioner or Physician Assistant - to join our Urgent Care team. As an urgent care provider, you will deliver timely, high-quality care to patients with acute illnesses and injuries - helping keep patients out of the emergency department while ensuring safe, efficient, and patient-centered care.

About the Role

In this role, you will function as a clinician, consultant, and trusted resource to patients and the care team. In collaboration with other care team members, you will evaluate, diagnose, and treat a wide range of urgent medical conditions while coordinating care and ensuring smooth patient flow in a busy clinical setting.

Key Responsibilities

  • Evaluate, diagnose, and treat a wide range of acute illnesses and minor injuries in a fast-paced urgent care setting
  • Perform focused exams, order and interpret labs and imaging, and develop evidence-based treatment plans
  • Perform common urgent care procedures within scope of practice (e.g., suturing, wound care, splinting)
  • Collaborate closely with physicians, nurses, and support staff to ensure efficient patient flow and high patient satisfaction
  • Provide clear patient education, discharge instructions, and appropriate follow-up care
  • Document care accurately and timely in the EMR while maintaining regulatory and quality standards

Required Qualifications

Education & Licensure

  • Nurse Practitioner: Master’s degree from an accredited nursing program
  • Physician Assistant: Graduation from an accredited PA program
  • Active Maryland license (or ability to obtain)
  • Current DEA license and Maryland Controlled Dangerous Substance (CDS) license
  • Current BLS certification (American Heart Association or approved provider)
  • Unrestricted license with no history of suspension or revocation
  • Eligible for participation with all payers

Skills & Experience

  • Prior experience in Urgent Care, Emergency Medicine, or Primary Care
  • Strong clinical judgment and commitment to evidence-based practice
  • Proficiency with electronic medical records (EMR experience preferred)
  • Excellent organizational, time-management, and multitasking abilities
  • Strong interpersonal, communication, and problem-solving skills
  • Ability to work independently while thriving in a team-based environment

Nurse Practitioner – Wound & Primary Care at Home | $15K Sign-On

Overview Meritus Health is building a high-autonomy house-calls program for patients who need wound care and longitudinal primary care in their homes and in our Home Health clinic. We’re looking for a Nurse Practitioner who wants to own their panel, practice at the top of license, and be the clinical backbone of this model. Role Summary In this role, you will provide in-home visits for wound care and primary care, with the potential for some clinic-based sessions at our Home Health department location. You will manage a defined panel of patients, coordinate with Home Health nurses and physicians, and have significant day-to-day independence in how you structure your schedule and care plans. Key Responsibilities Provide comprehensive in-home primary care: history, physical exam, diagnosis, and ongoing management of chronic and acute conditions. Deliver wound care in the home and clinic, including assessment, debridement (within scope), dressing selection, and application of advanced therapies. Develop and own individualized care plans for your patient panel, with an emphasis on preventing avoidable ED visits and hospitalizations. Perform periodic clinic sessions at the Home Health location for follow-up, new consults, and procedures best done in a clinic setting. Coordinate closely with Home Health nurses, PT/OT, and referring physicians to ensure seamless transitions and aligned treatment plans. Order and interpret labs, imaging, and other diagnostics; adjust treatment based on results and patient goals. Prescribe medications, wound therapies, and durable medical equipment within scope and state regulations. Educate patients and caregivers on wound care, chronic disease management, medication adherence, and preventive health. Document promptly and accurately in the EMR, including clear care plans and communication with the broader care team. Participate in case reviews/huddles with the Home Health and medical teams to review panel outcomes and refine protocols. Minimum 2–3 years of experience in primary care, home-based care, wound care, or similar high-acuity outpatient setting.
