Job Summary:

We are seeking an experienced and compassionate RN On Call Triage Nurse to join our team at Autumn Lake Healthcare in Bridgeton, NJ. As a member of our nursing team, you will play a vital role in providing exceptional patient care and support to our patients and their families. With a focus on clinical excellence and compassionate care, our team is dedicated to creating a positive and holistic healing environment.

About the Role:

As an RN On Call Triage Nurse, you will be responsible for providing high-quality nursing care to patients in a fast-paced and dynamic environment. Your duties will include:

  • Triage, assessing patients' conditions, and providing emergency care as needed

  • Coordinating care with other healthcare professionals and facilities

  • Developing and implementing individualized care plans

  • Providing education and support to patients and their families

  • Maintaining accurate and detailed records of patient interactions and care

Requirements:

  • Licensure: Current NJ RN licensure

  • Certifications: Basic Life Support (BLS) certification

  • Skills: Excellent communication and interpersonal skills; ability to work in a fast-paced environment; strong organizational and problem-solving skills

  • Additional Requirements:

    • Work Schedule: One week on, one week off

    • Vacation Time: 2 weeks PTO

    • Hospice Experience: A plus, but not required

    • Driver's License and Reliable Car: Must have a valid driver's license and reliable car
