Silver Stream Rehabilitation and Nursing Center is seeking compassionate and dependable Licensed Practical Nurses (LPNs) to join our nursing team. This position offers a consistent schedule of three 12-hour day shifts per week (7:00 AM–7:00 PM), including every other weekend, providing an excellent work-life balance while making a meaningful difference in the lives of our residents.

Responsibilities:

  • Administer medications and treatments as prescribed

  • Monitor residents' conditions and report changes to the RN Supervisor and physicians

  • Provide direct nursing care in accordance with facility policies and standards of practice

  • Accurately document resident care and services provided

  • Supervise and support Certified Nursing Assistants (CNAs)

  • Participate in care planning and interdisciplinary team meetings

  • Ensure compliance with all state, federal, and facility regulations

Qualifications:

  • Current LPN license in the Commonwealth of Pennsylvania

  • Long-term care experience preferred, but not required

  • Strong clinical, communication, and organizational skills

  • Ability to work effectively as part of a team

A Brief Overview Children's Hospital of Philadelphia is seeking a Licensed Practical Nurse for their Chadds Ford Primary Care office. This position will be Monday-Friday and will require one evening shift per week and a rotating Saturday shift. The shift will be based off the operational need. Two years of LPN and pediatric experience is preferred. The Licensed Practical Nurse performs routine nursing care of the ill, injured or infirm under the direction of a licensed professional (RN) nurse, nurse practitioner and/or a physician. The LPN participates in the planning, implementation and evaluation of nursing care in accordance with the scope of the LPN as defined by the State Board of Nursing and health system standards. Also supports operations through administrative duties as required. The nurse must demonstrate competence in caring for patients with moderate to complex health issues and assist the Site Manager with administrative issues which affect the clinical practices within the office as well as comply with the administrative policies and clinical protocols as outlined in policy and procedure manual. Administratively responsible to the Site Manager and Ambulatory Nursing Director for nursing practice issues.
