We're looking for a dedicated Nurse Practitioner to join our Program of All-Inclusive Care for the Elderly (PACE) team. What You'll Do

As a key clinical member of the PACE team, you will:

✅ Conduct comprehensive assessments and wellness evaluations

✅ Provide primary care management for participants with chronic and acute health conditions

✅ Develop and update individualized care plans in collaboration with the Interdisciplinary Team (IDT)

✅ Collaborate with specialists, hospitals, caregivers, and community resources to ensure coordinated care

✅ Educate participants and families on health conditions, treatments, and preventive care

✅ Participate in team meetings and contribute clinical expertise to participant care decisions

✅ Maintain accurate medical records and documentation

✅ Support quality improvement initiatives that enhance participant outcomes

✅ Serve as a trusted clinical resource for staff and caregivers

What We're Looking For

Required Qualifications

  • Graduate of an accredited nursing program

  • Current Registered Nurse (RN) licensure

  • Current Nurse Practitioner licensure

  • National Nurse Practitioner certification

  • Minimum two years of experience working with elderly or medically complex populations

  • Current CPR certification

  • Valid driver's license, reliable transportation, and proof of insurance

Preferred Qualifications

  • Geriatric Nurse Practitioner (GNP) certification with prescriptive authority

  • Two or more years of Nurse Practitioner experience in geriatrics

  • Experience in PACE, long-term care, home health, hospice, community-based care, or other interdisciplinary healthcare environments
