Hours:  24 hours/week, Night Shift, 7p-7a

Every third weekend

The Community RN Career Direct Care Pathway placement is comprised of four levels based on a RN’s minimum experience, education, and qualifications.

Level I - 0 – Less than 12 Months of RN Experience.

Level II - 12 Months - Less than 3 Years of RN Experience.

Level III - 3 Years - Less than 8 Years of RN Experience.

Level IV - 8 Years of RN Experience.

Job Profile Summary

This role focuses on providing professional and nonprofessional nursing care services in accordance with physician orders.  In addition, this role focuses on performing the following Clinical Nurse duties: Assesses, plans, implements, and evaluates the care needs of a designated group of patients or provides nursing care and identified clinical support tasks within the scope of practice for achievement of the patient’s plan of care as directed.  Provides professional nursing care to patients. Requires a nursing license.   A professional individual contributor role that may direct the work of other lower level professionals or manage processes and programs.  The majority of time is spent overseeing the design, implementation or delivery of processes, programs and policies using specialized knowledge and skills typically acquired through advanced education.

The RN Complex Care independently provides direct nursing care to patients and their families in accordance with established policies, procedures, and practices in a complex care area.

Job Description

Minimum Qualifications:

1. Massachusetts RN Licensure.

2. Current Basic Life Support (BLS) Certification.

3. Department Specific: Certification(s) relevant to the department and the type of patient care being provided may be required.

4. Experience based on level (I, II, III, or IV).

5. 1 year of RN experience required.

Preferred Qualifications:
1. Bachelor of Science in Nursing (BSN).

1. Participates in and maintains awareness of performance improvement initiates.

5. Uses professional nursing judgment to individualize the plan of care based on assessment of the patient's needs.

6. Manages own patient assignment by delegating and effectively based on patient needs and skills of team members.

7. Develops a comprehensive educational plan for the patient and family, utilizing appropriate resources, and documents according to department standards.

8. Recognizes patient/family rights through support of their informed decisions and advanced directives.

9. Continually evaluates and revises the care plan according to changes in the patient’s health status.

10.Develops a comprehensive discharge plan utilizing appropriate resources and referrals including community resources.

11. Supports the development of new staff members and helps facilitate their transition into unit. 

12. Guides the professional growth of department staff through precepting.

13. Participates in professional development by identifying learning needs and seeking appropriate education.

14. Participates in departmental committees as needed.

Physical Requirements: 
1. Occasional lifting of 30-35 lbs and turning patients.

2. Prolonged standing and walking.

3. Occasional twisting, bending, reaching, pushing/pulling, sitting, kneeling, and squatting.

4. Exposure to infectious disease and possible carcinogenic chemicals.

5. Exposure of blood-borne pathogens.

6. Requires fine manual dexterity to lift and maneuver patients.

7. Requires visual and aural acuity necessary to monitor patient care.

Skills & Abilities:
1. Ability to effectively communicate with patients, families, physicians, and healthcare team.

2. Knowledge of and application of the nursing process.

Requisition Number: R-23226 Facility: LOC0020 - 143 Longwater Norwell143 Longwater Drive Norwell, MA 02061 Department Name: SMC Surgical Services Status: Full time Budgeted Hours: 40 Shift: Day (United States of America) The Ambulatory Resource Nurse-RN provides clinical nursing care to patients commensurate with nursing licensure. Oversees and manages the department operational flow and staffing. Serves as first line of escalation for clinical staff. Escalates departmental issues through appropriate chain of command. Compensation Pay Range: $27.81 - $62.12 ESSENTIAL FUNCTIONS Responsible for the day-to-day flow within the department. Developing N ursing/MA assignment to be reviewed and approved by the nurse manager prior to distribution. Adjusts staffing as needed and adjusts N ursing/MA schedule , to ensure cover in the event of sick calls and emergencies. Adjustments will be reported to the nurse manager. Monitors nursing triage activity to ensure staff are working efficiently and escalates concerns to the nurse manager for review. Reports concerns and feedback as appropriate through the chain of command and will escalate: Building operation issues to building operations manager. Department operation issues to department operation manager. Clinical issues to department nurse manager. Communicates new workflows and policies implemented within South Shore Medical Center. Completes the medication and needle audit daily or delegates a staff nurse or medical assistant to complete this task. Completes review of the daily urine and throat culture log and pulling the report for review and distribution. Places self into the clinical role working as a staff nurse with an assignment. Day shift charge nurse completes warm hand off to evening charge nurse of any issues or concerns that had arisen throughout the day. Evening charge nurse to notify the lab and radiology when the last patient has left the clinic. Evening charge nurse to communicate any clinic exam rooms that require a deep clean to the building operations manager and/or the cleaning company email as appropriate when the last patient has left the clinic. Completes the fridge/freezer temp check/logs. Review required supplies and notify facilities of needs for the department clinic. Complete daily code cart check and weekly POC testing check. Other duties as assigned.
