Shift: Monday - Friday | 7:00 AM - 5:00 PM

Make a meaningful impact in the lives of pediatric and young adult patients with complex health care needs as a Home Care Nurse. In this role, you will provide skilled nursing care in the home, including patient assessments, medication administration, feedings, respiratory support, and assistance with daily living and therapy programs. Working under physician direction, you will collaborate with the care team to support coordinated care and may also assist with patient transportation.

Responsibilities

  • Deliver high-quality home health care in alignment with physician orders, care plans, and regulatory and payer requirements.
  • Coordinate closely with RN Case Managers and clinical supervisors to ensure safe, effective, and patient-centered care delivery.
  • Conduct skilled visits and monitor patient status, contributing to ongoing assessment and timely updates to the plan of care.
  • Educate patients, families, and caregivers on diagnoses, safety, and self-management to support better outcomes and independence.
  • Maintain accurate, timely clinical documentation and communication, including EMR updates and required reporting.
  • Collaborate with interdisciplinary team members to support comprehensive care planning and follow-up.
  • Participate in onboarding and training of new staff and contribute to a supportive learning environment.
  • Respond promptly and professionally to communications while actively engaging in team meetings and required learning activities.
  • Support operational efficiency by completing assigned duties and adapting to evolving patient and organizational needs.

Qualifications

Education:

  • Graduate of State Board approved program for Registered Nurses required.
  • BSN or BS in health related field preferred.
  • Masters education in health related preferred.

Experience:

  • Experience working in clinical healthcare position preferred.
  • Previous pediatric home care experience preferred.

License(s)/Certification(s):

  • Valid RN license in the state(s) of practice, including eligibility under a NLC multistate license required.
  • Dual licensed positions must obtain and submit proof of dual licensure within 90 days of hire/transfer; proof of application is acceptable during this period. 
  • Valid Drivers license and Maintenance of an acceptable driving record (i.e., no “OWI” convictions in last 7 years or no more than 3 moving violations in last 3 years) required.
  • Valid Auto Insurance required when driving personal vehicle.
  • Basic Life Support (BLS) certification required within 30 days through the American Heart Association.
  • IA Child Abuse Mandatory Reporter Training in state(s) where providing care required within 90 days of hire.
  • IL Child Abuse Mandatory Reporter Training in state(s) where providing care required within 90 days of hire.
  • IA Dependent Abuse Mandatory Reporter Training in state(s) where providing care required within 90 days of hire.
