About the Role

As a Certified Home Health Aide or CNA (Active-FL Licensed), you will play a vital role in our team, providing one-on-one care to our patients in their homes. Your duties will include assisting with daily living activities, bathing, dressing, grooming, and providing emotional support to our patients. Responsibilities

Provide personalized care to patients in their homes, focusing on their unique needs and preferences
Assist with daily living activities, such as bathing, dressing, and grooming
Monitor and report any changes in patients' conditions to our clinical team
Develop and maintain positive relationships with patients, families, and healthcare professionals
Maintain accurate records of care provided and patient progress
Collaborate with our interdisciplinary team to develop and implement patient care plans

Requirements

Active FL licensure as a Certified Home Health Aide or CNA
High school diploma or equivalent required; degree in healthcare-related field preferred
Minimum 1 year of experience as a Certified Home Health Aide or CNA; experience in home healthcare setting a plus
Ability to lift up to 50 pounds and perform essential job functions with reasonable accommodations
