About the Role

As a Licensed Nurse MDS Coordinator, you will play a crucial role in ensuring the highest quality of care for our residents. You will be responsible for coordinating the development, implementation, and maintenance of comprehensive care plans, as well as ensuring compliance with clinical and regulatory requirements. Your exceptional clinical skills, attention to detail, and strong communication skills will enable you to effectively collaborate with our interdisciplinary team to deliver exceptional care to our residents.

Responsibilities

  • Coordinate the MDS process, including conducting comprehensive assessments, identifying resident needs, and developing individualized care plans

  • Collaborate with the interdisciplinary team to develop, implement, and update care plans, ensuring alignment with resident goals and preferences

  • Ensure accurate and timely completion of MDS assessments, care plans, and other clinical documentation

  • Monitor and report on resident outcomes, identifying areas for improvement and implementing changes as needed

  • Participate in quality improvement initiatives and contribute to the development of clinical policies and procedures

  • Provide education and training to staff on clinical procedures, policies, and regulations

Requirements

  • Current licensure as a Licensed Practical Nurse (LPN) or Registered Nurse (RN) in the state of Missouri

  • Experience in a long-term care or post-acute care setting

  • Strong clinical knowledge and skills in gerontology, chronic disease management, and care coordination

  • Excellent communication and interpersonal skills

  • Ability to work effectively in a fast-paced environment and prioritize tasks to meet deadlines
