Responsibilities:

Administer medications, treatments, and therapies as prescribed by physicians
Monitor and report changes in residents' conditions, including vital signs and neurological status
Develop and implement individualized care plans in collaboration with the interdisciplinary care team
Provide education and support to residents and families on healthcare issues and treatment options
Collaborate with other healthcare professionals to ensure seamless transitions and continuous care
Participate in Quality Improvement initiatives to enhance resident outcomes and overall care
Demonstrate a commitment to excellent customer service and respect for the dignity and autonomy of each resident
