About the Role & Den Location

  • Facility Address: 1945 Roanoke Blvd, Salem, VA 24153 (Conveniently located near LewisGale Medical Center and the Roanoke Valley).
  • Setting: An intimate, state-of-the-art 6-chair dialysis suite built directly inside the post-acute rehabilitation facility.

DaVita is pioneering a better way to care for kidney patients. In our specialized Skilled Nursing Facility (SNF) Dialysis Dens, we bring the clinic directly to the resident. As a New to Dialysis (NTD) Registered Nurse, you will deliver high-touch, autonomous chairside hemodialysis care without ever having to worry about high hospital patient loads, grueling 16-hour on-call shifts, or chaotic ER turnover. You will build meaningful, long-term relationships with a consistent census of residents while benefiting from our industry-leading paid nephrology training program.

Comprehensive Paid Training (No Nephrology Experience Needed!):

Never worked in nephrology? No problem. DaVita provides an intensive 9 to 12-week paid dialysis training curriculum tailored specifically for Registered Nurses transitioning into the specialty:

  • Classroom & Simulation: In-depth training on kidney disease, fluid and electrolyte management, hemodialysis principles, and machine technology.
  • 1:1 Preceptorship: Side-by-side chairside mentorship with experienced lead dialysis nurse preceptors at our regional hub den (Highland Ridge).
  • Vascular Access Mastery: Hands-on competency in cannulation techniques, Central Venous Catheter (CVC) care, sterile dressing changes, and emergency access protocols.
  • Water Treatment & Safety: Complete training on Reverse Osmosis (RO) systems, water testing standards, and CMS/AAMI infection control compliance.

Key Responsibilities:

  • Autonomous Clinical Leadership: Serve as the clinical lead for the 6-chair den at 1945 Roanoke Blvd, overseeing treatment delivery, patient stability, and care coordination alongside skilled Patient Care Technicians (PCTs).
  • Patient Assessments & Monitoring: Conduct pre-, intra-, and post-dialysis assessments; calculate and adjust fluid removal (ultrafiltration goals); administer IV/dialysis medications, blood products, and saline rinses per physician orders.
  • Vascular Access Care: Initiate and terminate hemodialysis treatments via arteriovenous (AV) fistulas, AV grafts, and central venous lines (CVCs) using strict aseptic technique.
  • Interdisciplinary Coordination: Collaborate daily with facility nursing staff, attending nephrologists, physical/occupational therapists, and dietitians to ensure dialysis aligns seamlessly with resident rehabilitation goals.
  • Vigilant Safety & Compliance: Monitor vital signs and hemodynamic responses continuously, rapidly responding to complications (hypotension, cramping, access flow issues) and ensuring thorough EMR documentation.

Minimum Qualifications:

  • Education: Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) from an accredited school of nursing.
  • Licensure: Current, active, and unrestricted Virginia Registered Nurse (RN) license or valid Multi-State Compact RN license.
  • Experience: Minimum of 6–12 months of direct bedside nursing experience (Med/Surg, ICU, Stepdown, Telemetry, ER, Sub-Acute, SNF/LTC, or Rehab). No prior dialysis experience required.
  • Certifications: Current Basic Life Support (BLS) certification from the American Heart Association (AHA) or equivalent.
  • Clinical Skills: Strong physical assessment capabilities, sharp communication skills, attention to detail, and a desire to work in an intimate, autonomous clinical environment.

Why Choose DaVita DSD?

  • Work-Life Balance: Structured daytime schedules with no overnight on-call and predictable days off.
  • Competitive Compensation & Comprehensive Benefits: Medical, dental, vision, 401(k) matching, paid time off (PTO), tuition assistance, and clear clinical ladder advancement opportunities.
