About the Role:

As a Registered Nurse (Full-Time) with Aspire Senior Living Pleasant Hill, you will be responsible for providing exceptional care to our residents, including assessing their needs, developing and implementing individualized care plans, and administering medications. You will work closely with our interdisciplinary team to ensure that our residents receive the highest level of care and support.

Responsibilities:

Provide direct care to residents, including administering medications, performing medical procedures, and participating in mobility and rehabilitation programs
Develop and implement individualized care plans that address the unique needs and goals of each resident
Conduct thorough assessments of residents' physical and emotional needs, and communicate findings to the healthcare team
Monitor and report changes in residents' conditions to the healthcare team and physician
Maintain accurate and detailed records of resident care and services
Participate in quality improvement initiatives to enhance the quality of care and services provided to our residents

Requirements:

Current licensure as a Registered Nurse in the state of Missouri
Experience working with seniors and seniors with complex care needs

Manager Population Health Care Management - Registered Nurse

Job Description We are hiring a full time RN Nurse Manager to support our Population Health Outreach team. The Population Health Team within the Saint Luke’s Physician’s Group (SLPG) supports programs that foster the delivery of an innovative integrated care model across the Kansas City Metro and beyond. SLPG participates in one of Medicare’s new advanced alternative payment models known as Comprehensive Primary Care Plus (CPC+) and is rapidly engaging in other value-based reimbursement models. The Manager for Population Health Care Management provides oversight of day-to-day operations of the Care Management department, ensuring consistent implementation of evidence-based best practice interventions aimed at achieving efficiency, effectiveness, as well as patient, provider and employee satisfaction. This position reports to the Director for Population Health and works in close collaboration with other members of the SLPG Leadership team and the Population Health Team. The position is based at the Saint Luke’s System Office but will require frequent travel to the other Saint Luke’s Health System locations both metro and rural. This is a full time position working M-F 8 to 5.
