Job Responsibilities

Providing high-quality RN care in post-acute facilities
Reviewing various care options with the medical team, residents, and their family members
Coordinating with clinical teams to develop individualized care plans
Being accountable to follow and adhere to resident care plans

Provide clinical support and assistance to community team members as needed. Understand and follow infection control practices. Partner with the RCD and community team to promote infection control standards including any resident testing, training, and skills checks. Understand and practice the proper method of attending to and disposing of and the possibility of exposure to blood borne pathogens, bodily fluids, infectious waste, sharp sticks, and hazardous materials in compliance with universal care precautions.
