Supports the resident and family during transitions of admission, transfer or discharge including orientation and welcome to unit/room, packing/unpacking of personal belongings. (ESSENTIAL; DAILY) Encourages and assists resident in participation of activities and social programs, i.e.: parties, church services, outdoor programs, snoezlen. Regularly uses resident activity boxes. Offers timely and regular transport to activities. Demonstrates consistent reliability/dependability through attendance and punctuality.
