Jacksonville, FL 32257Under the supervision of the Triage manager and the direction of the Triage supervisors or Team managers, provides patient care in a variety of settings for the purpose of assessment, symptom management, treatment, teaching, emotional support, coordination of care, deaths, coverage of routine case management visits, etc. Work hours include every Saturday and Sunday from 8am-8pm and one weekday (either Monday or Friday) from 830a-5p, based on organizational needs. Primary Responsibilities: Weekend coverage: Performs skilled nursing activities such as: Completes assessment of patient for effective symptom management. Incorporates nursing skills, communication skills, and emotional intelligence skills to effectively assess problems/needs. Perform treatments and interventions in accordance with the plan of care and physician’s orders. Provide information and education regarding disease progression and anticipated needs at end of life. Provide instruction to the patient/care giver in accordance with the plan of care or changes in plan of care. Collaborate with patient, family/caregiver, and practitioner regarding goals of care and changes in treatment plan. Follows precautions based on plan of care. Provides appropriate emotional support during all conversations with patients/caregivers. Conducts follow up and collaboration calls as needs arise or as requested by the team. Weekday coverage: Act as a member of an assigned care team, coordinating resources and performing a wide range of skilled nursing activities for an assigned caseload of adult patients or to assist the wider team, as directed by the home or LTC team manager Conduct assessment of patient/caregiver needs; symptom management; teaching of appropriate care related to disease process; and participating as a member of the interdisciplinary group (IDG) in providing end-of-life care/support for patients and caregivers alike.
