This position is PRN field work as a Care Manager, assisting seniors with coordinating their care between multiple providers and agencies. Enable senior citizens to live comfortably at home and bring peace of mind to family members knowing their loved ones are cared for. Serve as the expert to assist client with obtaining providers to meet their needs. Visit clients in their homes to conduct initial assessments and re-assessments, write client care plans, recommend and coordinate changes in the level of care, assist clients with understanding available care options and connecting them with providers. Update care plan and supervisory notes in electronic records. New clients may at times require short notice assessments within the company service area of Fairfax County, Arlington, and Alexandria. Reports to the Administrator. Job Responsibilities Care Management, assisting seniors with varied needs to ensure their best quality of life. Duties depend on specific client needs, and may change over time. Accompany the client to doctor appointments, assist with coordinating client care between different providers. Make recommendations to client on what care options may best meet their needs, to include skilled care, therapy, PT, nutrition, and personal care. Assist the client with understanding and implementing guidance from health care professionals. Assist with keeping client family informed. Maintain careful documentation of client case. If desired, be available to assist with homecare client in-home assessments Required Skills & Qualifications RN with current license to practice in Virginia Certification as CMC, CCM, or other Care Management certification, or the requisite experience to obtain Care Management certification. Previous experience working in Homecare, Assisted Living, Home Health, Skilled Nursing Facility highly desirable.
