Position Title: Nurse Supervisor M-F 3-11pm Classification: Exempt Reports To: Director of Nursing (DON) / Assistant Director of Nursing (ADON) Position Summary In alignment with the organization’s mission to enhance the quality of life for residents, the Nurse Supervisor is responsible for overseeing and coordinating nursing care and services on their assigned unit/shift. This role ensures the safe, effective, and timely delivery of care, including medication administration, treatments, and documentation. The Nurse Supervisor provides leadership, supervision, and guidance to nursing staff while ensuring compliance with all applicable laws, regulations, and facility policies and procedures. Administer medications and treatments accurately and in a timely manner. Maintain accurate and complete clinical documentation in accordance with facility policies. Assist in the development and implementation of individualized care plans. Communicate changes in resident condition to physicians, families, and appropriate team members. Respond promptly and courteously to call lights and resident needs. Assist with admissions, re-admissions, and room transfers. Staff Supervision & Leadership Supervise Geriatric Nursing Assistants (GNAs), Certified Medication Aides (CMAs), nurses, and assigned staff. Monitor staff performance to ensure assignments are completed appropriately. Participate in employee performance evaluations in collaboration with Nurse Manager/ADON. Assist with staffing needs, scheduling recommendations, and workforce planning. Address and report employee concerns, performance issues, and disciplinary matters. Provide coaching, verbal counseling, and written warnings as appropriate. Foster a respectful, professional, and collaborative work environment. Communication & Coordination Give and receive detailed shift reports. Communicate effectively with residents, families, visitors, and interdisciplinary team members. Report unit concerns, incidents, and operational issues to nursing leadership. Participate in meetings, in-services, and training as required. Compliance & Safety Ensure adherence to infection control practices and standard precautions. Maintain compliance with all regulatory requirements (including DPH and state guidelines). Report hazards, equipment issues, and safety concerns immediately. Protect resident confidentiality and privacy at all times. Technology & Systems Utilize electronic medical records (EMR) and other required systems efficiently and accurately. Supervisory Responsibilities This position directly supervises nursing staff including GNAs, CMAs, Licensed Practical Nurses (LPNs), Registered Nurses (RNs), and other assigned personnel. Serve as the expert to assist client with obtaining providers to meet their needs. Visit clients in their homes to conduct initial assessments and re-assessments, write client care plans, recommend and coordinate changes in the level of care, assist clients with understanding available care options and connecting them with providers. Update care plan and supervisory notes in electronic records. New clients may at times require short notice assessments within the company service area of Fairfax County, Arlington, and Alexandria. Reports to the Administrator. Job Responsibilities Care Management, assisting seniors with varied needs to ensure their best quality of life. Duties depend on specific client needs, and may change over time. Accompany the client to doctor appointments, assist with coordinating client care between different providers. Make recommendations to client on what care options may best meet their needs, to include skilled care, therapy, PT, nutrition, and personal care. Assist the client with understanding and implementing guidance from health care professionals. Assist with keeping client family informed. Maintain careful documentation of client case. If desired, be available to assist with homecare client in-home assessments Required Skills & Qualifications RN with current license to practice in Virginia Certification as CMC, CCM, or other Care Management certification, or the requisite experience to obtain Care Management certification. Previous experience working in Homecare, Assisted Living, Home Health, Skilled Nursing Facility highly desirable. Highly proficient communicator, comfortable with various communication methods including phone, email, Zoom, telehealth, etc. Reliable transportation and willing to travel to client homes and appointments. Experience with electronic client record management systems. Flexibility and willingness to meet the unique needs of clients, caregivers, and family members.
