Ability to reference drug manual as needed. Relates well to residents, visitors and other staff members. Emotional and physical stamina to be of service to the residents in their daily living skills. Ability to practice self-control in speech, listening, body and facial mannerisms. Display a passion for caring for the elderly. Qualities of responsibility, diplomacy and ability to work well with fellow staff members Physical endurance for frequent activity Be able to occasionally lift boxes weighing 40 pounds. Specific Requirements: Must be at least 18 years of age. Prefer prior experience with medication assistance. Prefer previous experience, either professional, personal or volunteer, with the elderly population. The Personal Care Aide Duties include but are not limited to the duties listed below. Medication Assistance: Count Control Substance with outgoing PCA (sign your initials if count is correct). Report any missing controlled substances to administration. Prepare medication, bring medication to dining room during meal time, and assist residents with medication. If a resident requires medication prior to breakfast, PCA shall go to the resident's room and assist the resident with medication. If a resident requires afternoon or after dinner medication, the PCA shall assist the resident with medication from the med room. PCA must always watch residents ingest medication and properly document. PCA shall update medication administration record as needed, after Physician visits & ensure MAR & doctor's orders match PCA shall ensure that resident receives all necessary documents prior to doctor appointments. Consult and correspond with doctor offices for any necessary clarifications.. Record all pertinent information re: residents in the 24 hr. Log book. Personal Care Assistance: PCA shall assist residents in the dining room during mealtime by distributing meals to residents. PCA shall assist with cleaning the dining room after meals when required. PCA shall distribute facility provided snacks to residents during evening snack time. PCA shall assist residents with showering/bathing as needed. PCA shall assist residents with maintaining a healthy hygiene routine as needed. PCA shall assist residents by laundering residents clothing, linens, and towels as needed. Job Description Position Summary: Provide personal care and support services in the home for persons who are ill, disabled or terminal. Hospice Home Health Aide is assigned to Hospice patients by the Hospice Interdisciplinary Team, Registered Nurseor designee and functions under the direction of the plan of care developed by the Registered Nurse in an effort to maximize the patient's/caregiver's independence in performing activities of daily living and/ormaintain personal hygiene to and promote comfort in the case of hospice patients. Education, License & Cert: High School Diploma Or Equivalent Required. Preferred For NY. Requires valid driver’s license, reliable vehicle, and active vehicle insurance. Requires Basic Life Support certification. For NY : Home Health Aide Certification Required. If a caregiver has a Certification as a Nursing Assistant, then enrollment into Home Health Aide class will be arranged and covered by Guthrie Lourdes Hospice. Continued employment is dependent on successful completion and maintenance of the Home Health Aide certification. Until certified, the caregiver may shadow a preceptor per Hospice COP (Condition of Participation) guidelines. Home Health Aides must continue to meet the NY annual requirements to maintain their certification. Experience: Previous experience of one year in home care orskilled nursing facility preferred. Essential Functions: All tasks are patient specific and directed per the Hospice Home Health Aide Care Plan and may include: Assists patients with toileting. Prepares basic meals as necessary following patient prescribed diet. Assist patient with eating as needed. Turns and repositions patients in bed for comfort. Assists patients with ambulation, transfers, and exercises, utilizing appropriate body mechanics and assuring patientand caregiver safety. Performs simple procedures, i.e.: changing urinary drainage bag, changing simple dressings, following appropriate infection control procedures. Provides personal care and help with ADL’s under written plan, Documents care provided in accordance with the Hospice Home Health Aide plan of care in a timely manner. Records vital signs as directed. Performs light household tasks to ensure maintenance of clean, safe, and healthy environment, including changing bed linens. Promptly reports any change in the patient’s condition to RN / designee. Notifies RN of any patient / family concerns or questions. May direct and remind patients to take medication or treatments only as directed by RN. Follows universal infection control precautions with all patients and additional patient specific precautions (ie. Contact, droplet, respiratory) as appropriate. Responsibilities Resident Care Aide - Home Health Aide (HHA): Ability to assist residents with Daily Living Activities as needed that may include meal assistance, grooming, medication management, showering, dressing/undressing, toileting, skin care. Ability to use hands, arms and fingers in repeated motions that include ability to feel or operate objects, tools, or controls including grasping and turning motions. Ability to move and walk quickly and steadily in an emergency situation. Ability to assist with wheelchair transport of resident(s). Ability to apply/utilize Personal Protective Equipment (PPE) as required. Responsible for responding immediately to residents’ requests. Assists residents with Activities of Daily Living (ADL’s), according to their plan of care, individual needs and care preferences.
