Overview Oversees, supervises, coordinates, facilitates, and supports the Hospice After Hours care delivery activities, including field staff supervision. Carry out quality-driven and care coordination by communicating within the VNS Health interdisciplinary team and across providers and other resources to deliver the plan of care effectively and efficiently. Use agility and empathy to deliver continuity of patient care including coordinating supplies and providing education.
