Deliver personalized nursing and care management to patients in their home or care facilities. Constantly evaluate evolving patient needs and respond with plan of care adjustments. Qualifications Current license to practice as a Registered Nurse in New York State Minimum of one year nursing experience in a medical/surgical environment Valid driver's license required for all areas outside of Manhattan Wound Care Certification or willingness to obtain within one year of hire required Certification in Hospice and Palliative Care Nursing preferred Pay Range USD $118,942.00 - USD $152,717.00 /Yr.
