Able to adjust work and communication style based on situational needs Responsibilities: As an RN Field Supervisor, you will: Give and interpret the patient information documents i.e. admission packet, plan of care, advance directives, Bill of Rights, consents, patient education. Develop an individualized Plan of Care for the patient based on needs and findings of the nursing assessment. Reviews reports from all involved disciplined, re-evaluating needs and supervise all involved disciplines in the plan of care. Conducts on-site competency review as part of supervision activities. Prepare written documentation and summaries of the patient’s status and progress as needed for continuation of home health services. Document accurately and completely in the patient’s clinical record. Conference with all clinical and scheduling staff to assure continuity of patient care Perform patient re-assessments in the home. Reassess patient needs for services on a continual basis and discuss any changed in the plan of treatment with patient physician/family. Provide 24-hour on-call coverage on a rotation basis. Able to apply and maintain equipment according to the Policy and Procedure Manual. 20. Utilizes supplies effectively and to ensure cost containment. 21. Maintains integrity of the IV site while performing patient care. 22. Performs treatments/procedures according to specific standards (e.g., vital signs, patient weights, specimen collection, ostomy care [ileostomy, colostomy, urostomy/tracheotomy], catheterization [urethral, suprapubic], wound care, dressing changes, suctioning [oral, pharyngeal/nasal/trach], gastric tube [feedings and maintenance]. 23. Performs and monitors patient teaching according to the policy and procedures. 24. Coordinates care so that medications are administered as ordered by a physician and in compliance with the medication administration. 25. Coordinates the care team in order to facilitate a change in the level of care in compliance with the change in level of care standard. 26. Identifies the stages of death and dying. 27. Accurately and timely, completes plan of treatment and recertification as per Medicare guidelines. Completes OASIS SOC/ROC, OASIS Recertification, and Discharge Oasis per guidelines.
