JOB DESCRIPTION SUMMARY The hospice aide works under the supervision of a registered nurse and performs various tasks as assigned in accordance with the patient’s plan of care to meet the patient's personal needs and to promote comfort. The hospice aide is responsible for reporting and documenting observations and care performed. The hospice aide will be assigned in a manner that promotes quality, continuity and safety of a patient's care. ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES Responsibilities of the hospice aide include, but are not limited to, the following: 1. Performing personal care needs after given a patient's assignment and attending to his/her requests promptly. If unable to perform a certain task, report to the Case Manager immediately. 2. Job Summary: The self-directed Hospice and Palliative Care Public Health Nurse (HPCPHN) provides skilled nursing assessment, planning and coordination/care management across settings to patients in a private home, adult foster home, residential/assisted living care, intermediate care, skilled nursing facility, inpatient hospice facilities and hospital settings in order to maximize the comfort and health of patients and families consistent with hospice philosophy and CCS policies and procedures. Essential Responsibilities: Assume initial responsibility for assessing patient/family needs and consulting with attending physician, Hospice and Palliative Care Physician (HPCP) , other IDT staff members or other appropriate community or facility contacts as indicated by patient location and needs at the time of the assessment and care provision. Obtain data on physical, psycho/social, emotional and spiritual factors that may influence patient/family health status and incorporate that data into the plan of care. Facilitate advanced illness and/or end of life discussion with patient and family members to assist with choice of hospice or palliative care. Determine patient appropriateness for palliative care or hospice services (including hospice level of care and care site) in consultation with the HPCP. Case Management: assume primary responsibility for a patient/family caseload that includes the assessing, planning, implementing and evaluating phases of the nursing process; including scheduling visits to reflect patient care needs in order to maintain continuity of the plan of care; and advocate for patient and family. Participate as an active member of the IDT to coordinate information and services to update the plan of care. Maintain up to date patient records so that problems, plans, actions and goals are accurately and clearly stated and changes are reflected as they occur in accordance with CCS policies and procedures. Instruct primary caregivers, family members, employed caregivers and facility staff to provide hospice specific care as indicated. Attend weekly case conferences and actively participate in discussion of patient needs and care planning. About Legacy Hospice Services Legacy Hospice Services delivers high-quality, compassionate, home-based care to adults, children, and their families throughout the Portland metropolitan area and the Willamette Valley. We also provide hospice care in residential and assisted living communities, adult foster homes, and skilled nursing facilities - ensuring support and dignity for patients wherever they call home. Responsibilities Performs professional community health nursing visits and services for terminally ill patients and their families. Provides pain and symptom management tailored to patient needs. Serves as a member of the Hospice Interdisciplinary Team (IDT); coordinates and assists in implementing the plan of care for the patient and their family/caregiver. Completes nursing assessments and delivers palliative treatment and symptom management. May conduct initial visits to admit patients and initiate or revise the hospice plan of care. Prepares timely and accurate clinical documentation. Participates in IDT meetings and case conferences as appropriate. Collaborates with the care team to coordinate services and ensure continuity of care. Qualifications Education : As required by licensure. BSN or degree in progress strongly preferred. BSN may be required for specific assignments; experienced RNs without a BSN may be hired based on clinical experience with approval of the site CNO. Experience: One-year registered nursing experience or completion of either the Legacy Health RN Residency program or a nursing residency program that is equivalent in content, curriculum and duration. Hospice experience or one year medical/surgical acute care or long-term skilled nursing experience preferred. Skills: Working knowledge of and the ability to apply the basic principles and techniques of community hospice nursing in a variety of home settings. Must possess excellent clinical nursing skills and be able to utilize problem-solving techniques. Ability to establish and maintain effective working relationships with professional groups, volunteers and the general public. Must possess excellent communication skills both verbal and in writing. Keyboard skills and ability to navigate electronic systems applicable to job functions. Licensure Current RN license required. BLS for Healthcare Providers certification from the American Heart Association required. Valid driver’s license required. Certified Hospice and Palliative Nurse (CHPN) from the National Board for Certification of Hospice & Palliative Nurses (NBCHPN) preferred. Pay Range USD $54.37 - USD $81.21 /Hr.
